Greetings !

We, the 3rd year nursing students from Centro Escolar University, are presently in your
barangay. We would like to inform you that we are conducting a Community Survey at Saniboy
Health Center. The purpose of this survey is to identify existing problems, assess available
resources, and set priorities for health initiatives within our community.

To support our efforts, we are kindly requesting the organizational chart of the barangay to
better understand its structure and personnel roles. Additionally, we would also appreciate a
copy of the Barangay Profile to aid in our planning activities.
