1. INTRODUCTION
Based on scope of work, the MCHE Module 5 need to be decontamination, dismantling,
demolish and manage the scrap as per project direction. The MCHE currently located at
MLNG Car Park to be specific at Lot 39 Old MCHE Storage area.

2. SCOPE OF WORK

The following scopes shall be covered and abide to this document.

a) Mobilize and demobilize of equipment, machinery, material and tools.


b) Install perimeter metal hoarding.
c) Compact ground and strengthening for lifting activities.
d) Site Preparation on the temporary laydown area/ schedule waste area/ workers
shade/ toilet and TENORM decontamination area.
e) Survey and setting out work for bund wall, concrete slab and sump pit.
f) Install rebar and formwork for temporary bund wall, sump pit, and slab.
g) Concreting work for temporary bund wall, sump pit, and slab.
h) Housekeeping and reinstatement work.

6. GENERAL SAFETY & REQUIREMENT

a) All workers entering the worksite must be properly briefed on the rules and regulations
on the safety of working in the hazardous area.
b) Toolbox meeting shall be conducted prior to the start of any workday to ensure worker
are aware of the activity being carried out and safety requirement to execute the work.
c) HCSB to ensure that all PPE requirement are being adhered to.
d) The worksite must be clearly marked with suitable signboard to prevent any unauthorized
entry.
e) All personnel must observe strictly to the MLNG Safety regulation and occupational
safety, health and environment plan.
f) Precaution to be taken for lifting over live plant, field weld shall be strictly adhered to
PTW, JHA and relevant PTS.

7. TEMPORARY LAYDOWN AREA

a) Temporary laydown area shall be included perimeter hoarding, temporary bund wall,
temporary laydown cutting area, temporary covered TENORM decontamination area,
temporary scrap management area and workers shade with toilet and eye wash area.
b) The security personnel shall be provided at site gate to control the entry and exit of
personnel.
c) Lighting shall be provided during night hour.

8. WORK PROCEDURE

8.1. Mobilization and demobilization of Materials, Equipment and Tools


8.1.1.Equipment Mobilization Upon Entering MLNG.
a) HCSB shall ensure that mobilization activities are carried out based on
MLNG HSE and security requirement upon entering MLNG area.
b) Supervisor to ensure HCSB checklist to be fulfilled before mobilizing all
equipment to site.
c) All personnel must have valid MLNG body pass upon entering MLNG facility.
d) Prepare entry exit form at the security post and approved by MLNG.
e) Ensure the transportation is equip with spark arrestor and diesel driven
vehicles.
f) Supervisor shall inform MLNG person in-charge the plan for mobilization at
least one week before the work execution.
g) To ensure assign signalman / flagman during mobilization activities.

8.1.2. Electrical Equipment Inspections


a) All electrical equipment to be check by internal chargeman prior to
equipment inspection by MLNG representative to issue for approval for
usage at site.
b) Charge man shall engage by HCSB.
c) Refer to item no item 1.6 for the list of equipment and tools

6.1.3 Electrical Equipment Inspections

6.1.4 Material Mobilization


a) HCSB shall ensure material to be properly stored and secured before lay
at designated laydown area.
b) Supervisor shall monitor and arrange to barricade the laydown area and
install signage accordingly.
c) The laydown area must be approved by operation and notify area
owner.

6.1.5 Tool and Equipment Demobilization


a) Supervisor to ensure all the tools and cable shall be stored inside toolbox
and ensure to be locked before lifted to lorry crane.
b) All personnel responsible to check any remaining tools, equipment,
consumables, and material left at site shall be clear.
c) Supervisor shall fill up entry exit form prior to demobilization.
d) To ensure assign signalman / flagman during mobilization and
demobilization activities.

8.2. Setting out work


a) Setting out work shall be conducted by using open reel measuring tape.
b) Location of temporary bund wall, and sump pit shall be marked using timber peg.
c) The size of temporary hoarding and access gate shall be as per existing chain link
fencing and gate.

8.3. Install temporary perimeter metal hoarding and gate


a) Prior to install perimeter hoarding, at mobile crane operation area, ground shall be
compacted using 10t roller compacter if required.
b) Crusher run shall be levelled using backhoe and properly compacted to strengthen
the ground surface prior to start lifting work if required when the ground condition is
weak/damage.

c) Hoarding shall be as per location of existing chain link fencing.


d) Metals sheet shall be drilled to allow hole to fix with existing chain link fencing and
gate.
e) The height of the hoarding shall be as per existing chain link fencing approximately
2.3m.
f) The metal sheet shall be properly tie to existing chain link fencing using wire string
for every 1m-1.5m interval.

8.4. Install temporary sump pit


a) Sump pit shall be manually excavated by hand.
b) Concreting work for base shall be constructed after reached design location, depth
and size.
c) After base is harden, cement brick laying shall be started.
d) Plastering work on sump pit wall shall be after brick laying is completed.
e) Sump pit shall be constructed as per approve drawing that shall be submitted
separately.

8.5. Rebar and formwork installation


a) Formwork shall be soundly constructed, firmly supported, adequately strutted,
braced and tied to withstand the placing and vibrating of concrete, construction loads
and the effects of weather.
b) Formwork shall be clean and free from standing water, dirt, shavings, and chippings
or other foreign matter.
c) Level marker shall be installed at side of formwork and ensure gradient 1:20 for the
slab to sump pit.
d) BRC A2 wire mesh bars shall be kept clean and shall be free from pitting, loose rust,
will scale, oil, grease, or any other material which may impair the bond between the
concrete and the reinforcement.
e) Bending of the shapes indicated on the applicable drawings shall be fixed on site.
f) Wire mesh shall be accurately placed in the positions shown on the specific drawings
and held firmly during the placing and setting of concrete.

8.6. Concreting work


a) Before casting concrete, all oil, loose fragments of hard material, earth, mud, timber
and other debris shall be removed. Survey checks elevation, wide & length to confirm
pouring of concrete directly from mixer truck for area where mixer truck is accessible.
b) Concreting work shall be direct poured from concrete truck and wheel barrow when
required.
c) Protective covering (canvas sheets) shall be prepared in case of poor weather
conditions are encountered.
d) Vibrator shall be inserted into incompact concrete vertically at regular intervals and
withdrawn out of the concrete slowly without causing segregation.

e) The concrete surfaces were levelled and finished with steel/wood trowels before
initial set took place.
f) Water shall be used to cure the concrete after 24 hours and formwork shall be
removed.

8.7. Scaffold Installation


a) Scaffolding and platform shall be erected and covered at related areas that need to
be perform for sampling point of the TENORM and removal of the insulation sheet.
b) Scaffolding work shall be installed by competent scaffolder and inspected by
competent scaffolding inspector.
c) Dismantle scaffolding after cutting old MCHE body is complete.

8.8. Housekeeping
a) Clean up the job site.
b) Prevent development of safety hazards.
c) All equipment and tools used must be removed after use.
d) Store tools, equipment, and unused materials properly at the end of working day.

9. PERMIT TO WORK
a) MLNG e-PTW system will be diligently adhered to appropriate e-PTW forms will be
submitted to MLNG for approval prior to commencement of work ad copy shall be made
accessible and visible at work site.
b) JMS/JHA will be review and approved by MLNG prior to start work.
c) Approved PTW and related document will be displayed at site where it can be easily seen
and checked.
d) E-PTW shall be returned to receiving authority (RA)/ Area Authority Representative (AAR)
at the end of the day on daily basis for revalidation.
e) After completing the job and finishing 7 days duration, e-PTW form will be signed off by
Receiving Authority/ Area Authority Representative and submitted to MLNG filed for
further reference.
f) Work Leader shall be fully responsible to handle copy of e-PTW and for monitoring
activities to comply with JHA requirements.
