    I am writing to inform your good office for my application of maternity leave


    starting February 1, 2023 until April 30, 2023 (3 months). I am currently teaching as
    Junior High School Teacher at Mararag National High School, handling Mathematics
    Subjects. I am fourteen weeks pregnant, and my case is very sensitive since I
    experienced two consecutive pregnancy losses. Year 2017, I experienced ectopic
    pregnancy in my 2-months pregnancy and last year I experienced miscarriage in my
    2-months pregnancy also. I cannot afford to lose my baby this time. The difficulty of
    travel to Mararag National High School would be a big risk to my pregnancy.
    I am enclosing herewith my pertinent papers for my maternity leave e.g. Medical
    Certificate and Form 6.
