AFFIDAVIT OF DISCREPANCY AND CORRECTION

I, GLORIA ESPIRITU ESTRADA, surviving spouse Mario N. Estrada, of legal

age, Filipino Citizen, and resident of Brgy. Malimpuec West, Lingayen, Pangasinan,
herein represented by RONALD S. PALISOC, after having been sworn to in accordance
with law, hereby depose and state that –

1. That my late husband, MARIO N. Estrada is the registered owner of a parcel of

land situated in Brgy. Malimpuec West, Lingayen Pangasinan under the Original
Certificate of Title No. P-31085 issued by the Registry of Deeds of Pangasinan,
duly surveyed by Engr. Julius F. Reyes and covered by Tax Declaration No. 22-
0019-00020;

2. In the Original Certificate of Title No. 31085, the said parcel of land contains a
total land area of THIRTEEN THOUSAND SIX HUNDRED THIRTY ONE (13,
631) square meters while in the Tax Declaration No. 22-0019-00020, the
parcel of land contains TWELVE THOUSAND SIX HUNDRED AND THIRTY (12,
630) square meters;

3. That there is a discrepancy between the total land area in the Original
Certificate of Title No. P-3108 and in the Tax Declaration No. 22-0019-00020;

4. I am requesting the Municipal Assessor’s Office of Lingayen, Pangasinan to

resolve the said discrepancy and correct the land area indicated in Tax
Declaration No. 22-0019-00020;

5. I take responsibility for any outcome of correcting the same and shall not hold
any staff, officer, head of government agency-in-charge responsible; and

6. I am executing this affidavit to attest to the truth of the foregoing facts which I
have personal knowledge of and for whatever legal purpose this may serve.
