Safety Information
To the User of This Manual
The user of this manual is directed to read and carefully review the instructions, warnings and cautions contained herein prior to beginning installation or service activities.
While you may have previously installed or serviced equipment similar to that described in this manual, changes in design, manufacture or procedure may have occurred
which significantly affect the present installation or service.

Installation and Environment

Except for installations requiring certification by the manufacturer per federal standards, see that a radiation protection survey is made by a qualified expert in accordance
with NCRP 102, section 7, as revised or replaced in the future. Perform a survey after every change in equipment, workload, or operating conditions which might
significantly increase the probability of persons receiving more than the maximum permissible dose equivalent.

Diagnostic Imaging Systems - Mechanical-electrical Warning

All of the moveable assemblies and parts of this equipment should be operated with care and routinely inspected in accordance with the manufacturer’s recommendations
contained in the equipment manuals.
Only properly trained and qualified personnel should be permitted access to any internal parts. Live electrical terminals are deadly; be sure line disconnects are opened and
other appropriate precautions are taken before opening access doors, removing enclosure panels, or attaching accessories.
Do not under any circumstances, remove the flexible high tension cables from the x-ray tube housing or high tension generator and/or the access covers from the generator
until the main and auxiliary power supplies have been disconnected. Failure to comply with the above may result in serious or fatal bodily injuries to the operator or those
in the area.

Electrical-grounding Instructions
The equipment must be grounded to an earth ground by a separate conductor. The neutral side of the line is not to be considered the earth ground. On equipment
provided with a line cord, the equipment must be connected to properly grounded, three-pin receptacle. Do not use a three-to-two pin adapter.

Diagnostic Imaging Systems - Radiation Warning

X-ray and Gamma-rays are dangerous to both operator and others in the vicinity unless established safe exposure procedures are strictly observed.
The useful and scattered beams can produce serious or fatal bodily injuries to any persons in the surrounding area if used by an unskilled operator. Adequate precautions
must always be taken to avoid exposure to the useful beam, as well as to leakage radiation from within the source housing or to scattered radiation resulting from the
passage of radiation through matter.
Those authorized to operate, participate in or supervise the operation of the equipment must be thoroughly familiar and comply completely with the current established
safe exposure factors and procedures described in publications, such as: Subchapter J of Title 21 of the Code of Federal Regulations, "Diagnostic X-ray Systems and Their
Major Components", and the national council on radiation protection (NCRP) no. 102, "Medical X-ray And Gamma-ray Protection For Energies Up To 10 Mev-
equipment Design and Use", as revised or replaced in the future.
Those responsible for planning of x-ray and gamma-ray equipment installations must be thoroughly familiar and comply completely with NCRP no. 49, "Structural
Shielding Design and Evaluation For Medical Of X-rays and Gamma-rays of Energies Up to 10 Mev", as revised and replaced in the future. Failure to observe these
warnings may cause serious or fatal bodily injuries to the operator or those in the area.
Add additional safety information as needed
Only qualified and system trained Philips service staff is allowed to perform service (installation, maintenance, quality assurance) on the Brilliance CT systems. Make
TM

sure that the latest version of the service instructions are available. Read the corresponding instructions carefully prior to working on the system. For your own safety and
for more detailed safety information, refer only to the current version of the documents.
Use only specified tools and auxiliary materials. After finishing any service activity, ensure that all covers of the Brilliance CT, including the mylar gasket, are closed
TM

Precautionary Measures Regarding Electrical Voltage

Before you start the procedure outlined in this manual, make sure that you read and understand the warnings listed below.

WARNING PRIOR TO ANY SERVICE AND MAINTENANCE ACTIVITIES INSIDE COMPONENTS:

SWITCH OFF THE SYSTEM AT THE MAIN POWER SUPPLY, (USING THE ON-SITE ON-OFF
SWITCH) AND THE INTERNAL/EXTERNAL UNINTERRUPTIBLE POWER SUPPLY (UPS).
MAKE SURE THAT NO OTHER PERSON CAN SWITCH ON POWER OR SWITCH OFF THE
SECURITY MEASURES, WHEN INSTALLATION, MAINTENANCE OR SERVICE WORK ON THE
SYSTEM IS PERFORMED.

WARNING WHEN PERFORMING ANY PROCEDURE INSIDE THE PDC:

SWITCH OFF ALL EXTERNAL POWER, E.G. POWER FOR X-RAY WARNING LAMP.
PRIOR TO ANY INTERVENTION IN THE PDC, ALLOW AT LEAST FIVE MINUTES DISCHARGE
TIME AFTER THE LAST SCAN. HAZARDOUS VOLTAGE LEVELS (>500V) MAY BE PRESENT
EVEN IF THE SYSTEM IS SWITCHED OFF.

WARNING IN ADDITION TO THE WARNINGS LISTED ABOVE, MAKE SURE TO FOLLOW ALL SAFETY
GUIDELINES AS DESCRIBED IN THE SAFETY CHAPTER OF THIS MANUAL. FAILURE TO DO
SO CAN RESULT IN SEVERE PERSONAL INJURY.

Pulmo Instructions
• To plan two Pulmo Prospective scans using different scan modes (for instance "max inhale" and "max exhale")
perform the scans as follows:
1 Plan the first scan in the max. inhale mode and click Go.
2 When the scan is complete plan the second scan in the Max. Exhale mode.
3 Click Go.
NOTE Do not plan the two scans simultaneously.

• To avoid reconstruction failure due to invalid pitch, always keep the pitch values within 0.04 - 0.08 and the rotation
time 0.4 or 0.5 sec. The factory recommended values are: Pitch = 0.06 and RT = 0.5 sec.
• The Varian sensor is not supported. The only supported sensor is Bellows.
• Never edit the R tags on-line. After the scan and reconstruction are finished, end the study and only then perform R-
tag editing for the off-line reconstruction. If the peak is not identified use the "Add new tag" function. Editing the last
R-tag doesn't work.
• The message "Respiratory device is disconnected" may appear during the Pulmo scans, even when the device is
performing correctly. When this message appears make sure that the device is connected. If it is disconnected,
abort the scan.

Issues fixed in Brilliance V2.0.2/4.0.2

The following corrections and improvements were implemented in version 2.0.2/4.0.2:

• Annotations in MIP MasterCut

• LNA option – volume calculation accuracy
• BolusPro reliability improvements
• Fix patient data improvements
• HIS/RIS window is no more hidden behind other windows
• Cardiac viewer fixes
• DLP (Dose-Length product) display for short spiral scans accuracy
• Split-study is a key-enabled option
• Long TC crash on arcing
• Bug and Irregularities
• Reconstruction with SP Off
The reconstruction with SP off is now allowed. Although Philips recommends reconstructing with the SP on, in some
cases where the speed of the scan is crucial for the success of the scan, the SP may be turned off.

This applies to the Ultra Fast mode where the DFS is not used, and as a result the in-plane resolution decreases when
the SP is on and/or the pitch is higher than 1.

Consequently, in order to optimize the reconstruction quality in these scans, you may wish to turn-off the SP. However,
if the scan mode is Ultra-Fast and the needed scan speed is so fast that the actual pitch is higher than 1, the SP should
be on since the in-plane resolution will anyway be low.

To summarize the recommended SP status, see the table below:

Pitch UF Std. HR UHR

<1 OFF ON ON ON

(CIRS systems) ON
>1 ON ON NA
(IRS systems) NA

Brilliance (water-cooled) Version 2.2.1 and Mx8000 IDT V4.2.1

Application Release Notes

New Features in Brilliance 2.2.1

Cardiac
• New filters: Eight cardiac filters are available.
— CA and XresA are intended for large size patients
— CB and XresB are intended for Average size patients
— CC and XresC improves image quality for average size patients to get sharper images
— CD and XresD are intended for stent imaging
— The Xres filters use a special adaptive algorithm for noise optimization.
The user should select the best filter adequate for his needs. All 8 filters were present in the first and second betas of
BR2.2.1, however special attention should be given to image quality with filters CC and the Xres filters.

• Cosmix filter: Four new filters are introduced in BR2.2.1. The purpose of the COSMIX filters is to reduce the noise in
the vessels while preserving their edges thus enhancing the coronaries visualization in cardiac applications.
Validation of these filters will be done both internally and externally (Not in beta 1)
• Burlap artifact: A correction to reduce the burlap artifact on cardiac images is introduced to BR2.2.1. The validation
of this artifact will be done mostly internally.
• Continue cardiac scan: If scan continued, one ECG file and one volume scan is resulted when reviewing the data in
CT viewer or Cardiac viewer. Not applied to off line.
• Irregularities detection and correction: An automatic algorithm to detect irregularities in the patient heart rate runs
in the background of a cardiac scan. In case such an irregularity is detected, the following occurs:
— If the Cardiac Dose Right was ON, it will be cancelled automatically to allow reconstruction of additional phases at
higher image quality. The user receives the following message: “Cardiac Dose Right cancelled due to unstable
heart rate”.
— The ECG viewer notifies the user the number of irregularities detected
— The ECG wave presents the first irregularity detected.

CSIP Level 1 © 2008 Koninklijke Philips Electronics N.V All Rights Reserved Refer to Front Cover 111
V2.2.1 / 4.2.1 Upgrade Instructions Application Release Notes

— The irregularities detected are automatically labeled with a question mark. The user can accept or reject the
automatic detection with the left-click options. Before Accepting / Rejecting a detected irregularity, the user should
review the ECG wave and determine if the detected irregularity is actually an irregularity.
— If the user accepts the irregularity the question mark will disappear and in some cases additional phases will be
automatically added to the reconstruction request.
— If the user rejects the irregularity: the label will be removed and no changes in the reconstruction procedure will
be made.
— If the user takes no action (neither accepts nor rejects the irregularity): the question mark label remains and no
changes in the reconstruction is made.
— In offline reconstruction or in re-recon the user has three possibilities:
— Re-run the irregularities detection algorithm: right-click on the ECG viewer
and click Run Irreg autodetect
— Accept or Reject irregularities

— Click Reset

• Recon parameters (such as FOV and Shifts) are saved after evolving.

New Filters (applicable only for system with CIRS)

Six new filters with improved spatial resolution (both in plane and longitude direction) are introduced, two for each scan
mode: YA and YB for Standard Resolution, YC and YD for High Resolution and YE and YF for Ultra High Resolution.

mA and mAs Values

The mA values now appears together with the mAs value; this allows the user to know what tube power was used for a
given scan. In order to enable the display of both mA and mAs values:
1 Click Preferences in the Archive toolbox.
2 Select Image Viewing | Titles in the Preferences tree on the left side of the window.
3 Select (check) the Current checkbox.

Bolus Pro Automatic ROI Graphic Improvement

• Operator adjustment of the automatic ROI is limited to less than 1.5 around the original position to avoid inaccurate
positioning.
• Active ROI (the one that will trigger) is more prominent.

A New Surview Mode

A new Surview mode is introduced called Multi Surview. In this mode a large number of Surview scans may be
performed within the same study. This enables rescanning after a scanner operation errors or when a repetitive
procedure for Oncology purposes is needed.

In order to enable this new mode open the Preferences | Scan Options and select Multi Surview in the Surview
Mode drop-down list.

Force Max FOV in Detailed and UH Resolutions

Max FOV on the plan viewer includes shift calculations.

New GUI in Plan VIEWER

Max FOV is Displayed on the image. Data for off-line planning: the user will know that he cannot use data beyond the
max FOV.

Minimum Delay in Timed Scan

A new parameter was added to the scan protocol, in the injection tab: Minimum delay. This parameter keeps the scans
with the minimum possible delay.

General
• After installation, make sure that there all protocols are correct.
• In Preferences, clear the D-DOM checkbox.
• It is highly recommended not to have more than 400 studies in the Local device Archive.
• When logged on as CT it is not possible to use the Long Tube Conditioning application.
• After restarting the host, Windows may launch with the wrong resolution (640X480), and without dual monitor
horizontal span support (both monitors will have the same display). In this case, restart the system again.
• If you move the bed manually by using the tapeswitch (either footswitch or couchswitch) while the system is
initializing or ready for a scan, the system will return to plan scan and numerous error message will appear. Close the
messages and continue routine work.
• Do not use a pitch higher than 0.6 in UHR scans. To avoid errors, always use appropriate Scan Type when scanning
with UHR mode.

Scanning
• After pressing Pause during Repeat Scan, clicking Next Series will perform the cancelled scan (repeat or continue).
• When pressing pause on the CT-box, the Examination Flow dialog box is displayed. If Continue is then pressed, the
continued scan may start a couple of slices prior to the last one, meaning that some slices may be overlapped and
have the same image number.
• If the End Study is clicked before the reconstruction ends, the Surview ES image will not be saved.
• If the Start Study button does not respond, try clicking Home and then click Start Study again. If the problem
persists, wait 1 minute and try again.

Plan on Surview
Do not plan Autolaunch to a review/analysis application in more than one step. Bear in mind that planning MPRs is an
Autolaunch by itself.

Archive
• To see new studies in the local archive, click Refresh in the Directory window.
• Filtering the series list: when typing a partial string, use "*" before and/or after the string. For example: searching for
patient named Smith, type the partial string Sm*.

• When copying data to the EOD, select 1-4 studies a time. Copying large batches of data may cause the system to
slow down.
• Avoid using the Full Rebuild option in the Archive.
• It is recommended not to delete more than 25 studies at once.
• Do not use the Split Series feature in Sub-selection.
• If series' appear to be missing in the IRS device, switch to a different study and then back to the current one.

Bolus Tracking
• If Go is disabled, check that the tracker post threshold delay is legal.
• Do not use minimum cycle time when rotation time=1.

Interventional CCT
• CCT fluoro - The last few images may not appear in the CCT viewer when releasing the pedal. These slices will
arrive after pause.
• Pressing the Pedal while scanner is initializing may interrupt the initialization process. Press the pedal only when
"Press Pedal" message appears.
NOTE If the table height and tilt are changed during the scan, image parameters will not be updated.

Pulmo
• Do not use Pulmo scans in combination with Bolus tracking, timed scans or cardiac scans.
• Start offline recon only after the Pulmo study is completed and closed.
• The Pulmonary viewer in the operator console may not be accurate and used for the reconstruction process only.

Cardiac
• Generate a Cardiac protocol only from a factory Cardiac-protocol.
• Do not scan patients with a heart rate lower than 40 or higher than 140.
• Y Scale of HR graph does not adjust automatically. This results in the graph running out of the visible area.
• ECG R-tag Correction: Do not use APB/VPB correction

V2.2.1 / 4.2.1 Upgrade Instructions Application Release Notes

• ECG Editing tool: Do not use "Add new R-Tag" option.

• Do not use the following options during a cardiac study:
— Tilt
— DOM (in Retrospective Cardiac scan)
— Auto Processing
— Cardiac Injection Guide
• Do not delete ECG from the archive.
• It is highly recommended to use 0.2 pitch for all heart rates
• Automatic arrhythmia detection is applied before and after each scan. Currently, correction of arrhythmia can only be
performed manually via R-tag editing.
• Do not use off-line reconstruction from the Cardiac Viewer (Re-Recon)— perform additional reconstructions in the
Off-Line Recon application only.
• When Cardiac Doseright is applied, automatic arrhythmia detection will automatically cancel this feature if an
irregular rhythm is detected during a scan.
• Reset monitor between scans.
• Prospective gating for calcium scoring should be performed at phase 75% (and not 85%). Prospective and
retrospective phase definitions are now equivalent.
• Do not perform a non-cardiac scan after a cardiac one in a single study.
• Do not use the following filters: 'D', 'CD', and 'XCD'.

Cardiac Off-line Reconstruction

The reconstruction shift and zoom settings are not saved with the Raw Data in Cardiac Scans. Therefore, if off-line
reconstructions are needed (e.g. for different phases) proceed as follows:

1 Load the original series in the CT Viewer and select the 2D scene.
2 Make a note of the x and y shift and the zoom settings of the images.
3 Load the Raw Data from the IRS.
4 Change the shift and zoom values from the original images (see Step 2 above) and then start the off-line
reconstruction.

The actual cycle time can differ from the planned cycle time by up to 0.5 seconds.

Viewing and Filming

• When sending images from CT Viewer or from Cardiac viewer to film, the contrast sign (C) will not appear on the
filmed image. The ECG value in cardiac images will also be missing.
• Before using Generate Protocols or Air Calibration, Click Start Study/End study and make sure the patient data
fields are empty.
• Avoid printing while images are being loaded to the Master Film.
• When using Auto Filming, always make sure all requested images were loaded to Master Film with correct
windowing settings.
• Dental CT Application: only images with matrix 512x512 can be loaded to the application. Use only Spiral protocols
with SP reconstruction.
• Stereotaxis: use only Spiral protocols with SP reconstruction or Axial protocols. If axial protocols are used, select
only 16x0.75 and thickness 3mm or 16x1.5 and thickness 6mm.
• Cursor coordinates value on surview may not be accurate, do not use them to measure the positioning.
• Do not use the CT / MR application.

Common IRS (CIRS) Guidelines

• Evolving reconstruction might be slow. You will see the green line advancing and images still arriving even when the
scan is completed.
• The toolbox state messages might not be synchronized (messages such as: Scanning, Reconstruction, Acquisition
complete).
