CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF SERVICE is entered into by and between the following:

JIKA FARM, a private, TESDA registered training institution existing under and by
virtues of the laws of the Republic of the Philippines with principal address at PUROK 4,
SIMPAK, LALA, LANAO DEL NORTE, represented by the Farm School President or
Administrator, ALLAN M. ARNOCO, hereinafter referred to as the INSTITUTION;
- and -

MELJUNE M. RABACA, of legal age, Filipino, single, and a resident of Simpak, Lala,
Lanao del Norte, Philippines, hereinafter referred to as the TRAINER;
- WITNESSETH that -
1. The TRAINER shall make available her services, knowledge and skills to the
INSTITUTION and agrees to undertake the Scope of Services for the following:
a. Undertake and carry out practical on-site training for the scholars
nominated by the INSTITUTION for the agriculture-related trainings;
b. Make recommendations regarding discipline and proper behavior and
determine the proficiency of the scholars in specified trade or craft; and
c. Perform such functions or tasks as may be necessary for the
improvement of the scholars’ skills, knowledge and attitudes.
2. The TRAINER understands and agrees that all records and documents acquired
during the agreed period of service shall be the property of the INSTITUTION
and she shall not disclose or divulge any such information or data without the
prior approval of the Farm School President or Administrator, even after the
termination of this CONTRACT OF SERVICE;
3. For and in consideration of the Services to be rendered, the INSTITUTION shall
compensate the TRAINER fifteen percent (15%) or with total amount of One
Hundred Eighty Thousand pesos (180,000.00) for the conduct of training title
FARMERS FIELD SCHOOL(FFS) ON PRODUCTION OF HIGH-QUALITY INBRED
RICE AND SEED CERTIFICATION , AND FARM MECHANIZATION.
4. The TRAINER shall not engage in any activities related to the above-mentioned
training or any trainings whatsoever with other institutions while the training is
on-going or not yet completely turned-over;
5. This CONTRACT OF SERVICE is project-based and will cover only on the
specified training dates stated below with schedule of training attached; and
6. This CONTRACT OF SERVICE shall be effective from August 29, 2023 to
December 31, 2026.
 IN WITNESS WHEREOF, the parties have hereunto set their hands this _____ day of
