Sub: Urgent Attention Required – Deviations in Tie-Down Works at P4-14 P1, B1 & C1
(Drawing No. 2363/DSN/TW/D/083-R2)

We are writing to bring to your immediate attention a matter of significant concern


regarding the tie-down works at P4-14 P1, B1 & C1, as outlined in Drawing No.
2363/DSN/TW/D/083-R2 (Letter no GMRCL/AMD-C1/Afcons/1511 dated 19-Aug-2023).

Upon conducting a thorough inspection, our team has identified several observations that
require your urgent attention and corrective action. The following deviations from the
approved drawing have been observed:

1. Undersized Top and Bottom End Plates:

The top and bottom end plates are not in accordance with the Submittal Drawing
specifications (As per Drw. 200 x 200 x 60), immediate corrective measures are
necessary.

2. Sub-standard Spreader Beam:

A spreader beam of insufficient size was utilized for tie-down, contrary to the
specified drawing (as per Drw. ISMC 300). This poses a potential risk and must be
rectified promptly.

3. Improper placing of PT (Prestressed) Bars:

The PT Bars were not fixed as per the Submittal Drawing, and their Improper
placement requires immediate attention to ensure structural integrity and Safety.

4. Use of Sub-standard Cross Beams:

Nonstandard cross beams were used for tie-down at the P4-14 P1 segment, This
non-compliance needs to be addressed promptly.

With respect to above issues / Observation (photographs attached), GEC/GMRCL strongly


advises that, prior to any erection works, Requests for Inspection (RFI) must be raised with
attached checklists and TPI reports as required and approved by GEC/GMRC/Competent
Authority. This is a mandatory step to ensure compliance with project specifications and to
avoid further discrepancies.

Your immediate attention to rectify these above observations and implement the necessary
corrective actions.
