In continuation to the above ref. Latter No 3 (GEC-GMRCL-PH-2/PKG-C1/2023/1431, Dated


29-Sep-2023), some critical issues that have been identified during the extradosed casting
Yard inspection, it is imperative that prompt and effective actions to be taken to rectify these
concerns to ensure the overall quality, durability, safety, and integrity of the structures.

1. Segment ID P4-15 A2: Concrete Spalling (NCN no-36)

During the inspection, we have observed a significant issue with the concrete at the bottom
of the soffit on Segment ID P4-15 A2. There is clear evidence of a patch, Concrete damage
while inserting PT bars, which raises serious concerns about the structural integrity and
durability of the segment. This issue has been identified under Non-Conformity Notice
(NCN) no-36.

Immediate action is required to investigate the root cause of the concrete spalling and to
implement necessary repairs. Please provide a detailed report outlining the cause of the
spalling, along with a comprehensive plan for rectification. The timeline for these corrective
measures is of utmost importance, and we expect a swift response from your team.

2. Segment ID P4-15 B11: Major Honeycombs due to poor workmanship:

Another critical issue has been identified at Segment ID P4-15 B11, where major
honeycombs have been observed. This poses a significant risk to the durability segment and
structural integrity of the segment. It is imperative that necessary action to be taken
promptly to address this concern and prevent any further deterioration.

We require a comprehensive report on the extent of the honeycomb, the root cause analysis,
and a detailed plan for remediation.

GEC suggest you understand the urgency of this matter and allocate the necessary resources
to ensure the swift resolution of the problem.

Your immediate attention to the above matters is crucial to mitigate potential risks and
ensure the successful progression of the project.
