AFFIDAVIT OF HEIRSHIP

I, MARIEFE FOLLOSO SEMENIANO, of legal age, and a

resident of L-01 B-16 Kalayaan 2 Kings Point Bagbag, Quezon City, after
having been duly sworn in accordance with law, hereby and depose and
say:
1. That I am the surviving heir of the late Chresajoy Semeniano
Salditos who died on September 25, 2023 in Sta. Maria
Bulacan;
2. Our legal relation to the deceased can be evidenced by
competent public official documents such as Birth Certificate;
3. That during her lifetime she was a member of an insurance life
plan;
4. That Chresajoy Semeniano Salditos died in state sometime in
September 25, 2023 leaving without any testamentary wil nor
debts not settled until present and upon her death she was
survived by her mother and only compulsory heir namely,
MARIEFE FOLLOSO SEMENIANO;
5. We are executing this affidavit in good faith to attest to truth
that there is no other heir or heirs except for the above
mentioned heir of said deceased daughter and for whatever
purpose it may serve.
