A1MC Group assignments for Lecturer Assessment 1 (LA 1) January 2022

Presentation on the following cases; summarize on the lessons learnt and


identify the root cause/s. (3 candidates per group. The groups are based on the
attendance list. Maximum 10-minute video session inclusive of Q & A ).
Class submissions presentations will be in week 3 & 4.

Preparations: language, structure etc. – 20 marks


Need to submit (group’s basis) written report (not less than 1 000 words / Font is
Arial 11) of the case prior presenting. No need to do “comb binding”. Except the
track of vessels, no other picture is required in the report. The summary must
include all main elements / items in the official report.

Presentations:
Preferably by video
Vocal projection, delivery skill, salient points, accuracy of facts, time- keeping,
fonts used, delegation and summarizing - 15 marks

Participations – 15 marks
Need to provide key points (by the listener)

Common root causes in previous maritime accidents:

1. In breach of procedures.
2. Do not maintain proper lookout.
3. Do not proceed at safe speed.
4. Do not adhere to industry’s guidelines.
5. Lack of proper passage planning.
6. Lack of communication.
7. Environmental factors.
8. Equipment failures.
9. Inability to operate the equipment / system correctly.
10. Misunderstanding in the limitation of the equipment.
11. Complacency.
12. Unawareness of the “distraction factor”.
13. Providing a “false sense of security”.
14. Fatigue.
15. Human factor.
