AFFIDAVIT OF UNDERTAKING

I, Cosme Ducay, of legal age, Filipino with address at Centennial Road Sitio
Kalamias Putol Street San Isidro Cabuyao City do hereby undertake:

1. That I shall surrender the Chassis with MV no. 0301-00000030064 that was
attached to the tractor head involved in an accident in Maharlika Highway
Brgy. Minuli, Carranglan Nueva Ecija on December 20, 2023 if required to
be presented before the Authorities or Court of proper jurisdiction upon
proper summons or upon failure to arrive at a compromise agreement with
the heirs of the deceased;
2. That I shall also surrender and participate in all proceeding in relation to the
abovementioned accident upon proper summons;
3. I am executing this instrument for whatever legal purpose it may serve.
