City, after duly sworn to, in accordance with law, hereby depose and say:

1. That I am a holder/owner of a Philippine Passport;

2. That sometime on year 2019 the said Philippine Passport was lost or misplaced;

3. That all exert to retrieve, and recover, said Philippine Passport proved futile;

4. That I am executing this affidavit to attest to the truth of the foregoing facts and for whatever

January 24, 2021 in Metro Antipolo Hospital, Antipolo City, registered and lawful
owner of a Memorial Lot Title identified as Block 28 Lot 4 of Deed of Sale No.
1116 at Holy Gardens Memorial Park, Manila East Road, Brgy. Dolores, Taytay
Rizal;

2. That the deceased MELBA M. VILLAROSA was single;

3. That the said Memorial Lot Title has been lost/missing where the place she kept it;

4. That I searched time and again every place where it might be but my efforts have
been futile;

5. That I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever legal intents and purposes this may serve.

7. That the said original copy of Certificate of Crypt has been lost/missing where the
place I had kept it;

8. That I searched time and again every place where it might be but my efforts have
been futile;
