AFFIDAVIT OF LOSS

I, Nodicakes Qerobs, of legal age, married, Filipino citizen, and a

resident of Bagumbayan, Tuguegarao City, Cagayan, after having been duly
sworn to in accordance with law, depose and state:

1. That I am the holder and in possession of a Transfer of Certificate

of Title No. 143-1999721452, which cover a parcel of land
consisting of an are of One Thousand Three Hundred Sixty Nine
(1,369) square meter, located in Barangay Pengue, Tuguegarao
City, Cagayan (Lot No. 210 PSD-05-112140), which was acquired
by my late father, Chrissy Boi Qerobs, duly recorded as Doc. No.
112, Page No. 69, Book No. XI, Series of 1980 in the Notarial
Registry of Glenny Boi Pasca Jr., Notary Public of Tuguegarao
City, Cagayan;

2. However, the abovementioned title was destroyed when my house

in Bagumbayan, Tuguegarao caught on fire last November 22,
2023.

3. That by reason of the above fact and circumstance, and considering

that the said title is already beyond recovery, it is considered as lost
for all intent and purposes;

4. That as the owner or said parcel of land. I am executing this

Affidavit of Loss for the purpose of Reissuance of Title by the
Register of Deeds in accordance with Section 109 of P.D. 1529 or
the Property Registration Decree;

5. That I am executing this Affidavit of Loss to attest to the

truthfulness of the foregoing and for whatever legal intent and
purposes this may serve.
IN WITNESS WHEREOF, I have hereunto set my hand this 7th day of
December 2024, at the City of Tugueagarao, Cagayan Valley, Philippines.
