Subject: Dropped Object Involving Stabbing Guide Assembly on PH-100 Pipe Handler

Product: All TDS-8SA, TDS-8GB and TDS-1000 Top Drive Drilling Systems

Affected
Assemblies: Clamp Cylinder Assembly, PH-100 Pipe Handler (P/N 30125050)

Objective: Advise customers of dropped object involving PH-100 Stabbing Guide, of


recommendation to inspect all affected assemblies, and of available safety
lanyard upgrade kit.

Issue: NOV received a recent report in which two of the Hex Head Cap Screws (P/N
50012-16-C5D) that mount the front guide of the Stabbing Guide Assembly
sheared and fell to the rig floor. The third hex head cap screw was not found.
The rig was performing reaming operations at the time and was experiencing
heavy vibrations. No injuries occurred as a result of this incident

NOV had received a previous report in 2013 in which the front guide of the
Stabbing Guide Assembly (P/N 30125159) fell and impacted the rig floor. The
rig reported that they were drilling the top hole and were experiencing heavy
vibration in the TDS-8SA Top Drive. No injuries occurred as a result of this
incident.

The Stabbing Guide Assembly is located on the bottom of the Clamp Cylinder
Assembly on the PH-100 Pipe Handler and aligns the drill pipe and saver sub
when making up connections. The stabbing guide is a two piece assembly and
each guide is attached to the Clamp Cylinder Assembly with three 3/4-10 UNC
hex head cap screws, lock washers and safety wire.

Solutions: NOV strongly recommends that customers perform post-jarring inspection on
their top drives prior to, as well as following jarring or hard drilling, as outlined in
the Top Drive Service Manual and in Product Bulletin TDS-93-01.

When changing to different drill pipe sizes, care must be taken to use the correct
stabbing guide with suitable O.D. range as well as the dies in the Clamp
Cylinder Assembly. Using incorrect die/stabbing guide combination can cause
the full clamp force to be imparted into forward stabbing guide.

NOV has developed an upgrade kit which adds a lanyard assembly to retain the
Stabbing Guide Assembly to the Clamp Cylinder Assembly (See Figure 1).
