Subject: Collision between travelling assemblies and service access basket.

Product: SAB-90 Service Access Basket.

Affected
Assemblies: All equipment with controls and feedback devices.

Objective: Inform customer of the risks associated with unauthorized modification of

NOV drilling equipment.

Issue: NOV have been advised of an incident that occurred when a top drive
collided with an SAB-90 Service Access Basket resulting in a fatality on an
offshore drilling rig. Initial findings identified that a positioning sensor had
been modified causing incorrect feedback to be passed to the control
system. This modification resulted in an unsafe condition when lowering
the travelling assemblies whilst the SAB-90 stabbing basket was in
operation.

Recommendation: Alert users that any unauthorized modification of sensors or feedback

devices or any attempt to physically bypass such devices is strictly
forbidden and exposes personnel and property to danger.

Customers should check that all feedback devices and sensors are
functioning correctly prior to operating equipment per guidance within
product manuals and implement corrective actions where necessary. If
there are any concerns regarding the function of any sensor or feedback
devices, customers should contact their local service center for advice and
further recommendations.
