Subject: Potential Dropped Object Involving Block Dolly Roller Bracket Shims

Product: All NOV Top Drive Drilling Systems

Affected
Assemblies: Block Dolly Assembly (Multiple Part Numbers)

Objective: Advise customers of a recent incident involving bolts that loosened on a Block Dolly
allowing movement of the Roller Bracket Shims and to reiterate correct installation of
shims.

Issue: NOV received a recent report in which the bolts on a Block Dolly Roller Bracket
Assembly (P/N M614004991) loosened while secured with lock wire, resulting in
movement of the Roller Bracket Shims (P/N 115730). When the bolts are not
adequately torqued and the slotted shims are not oriented properly, there is a potential
for the shims to fall to the rig floor. There were no injuries reported as a result of this
incident.

Solutions: NOV recommends that all bolts on the Block Dolly Roller Bracket Assembly be
inspected to ensure they are fully torqued and lock wired properly. Also, check that the
Roller Bracket Shims are installed either from the top or the side of the assembly as
shown in Figure 1. Note: The slotted shims should never be installed from the bottom.

Failure to follow the recommendations and/or guidance in NOV Product Bulletins may result in
death, bodily injury or property damage.

Replacement of damaged and worn parts is important to rig safety and operation. Operators should
also refer to previous NOV Product Information Bulletins and Safety Alerts at
http://www.nov.com/drilling for additional information related to this issue and information regarding
safe operation, maintenance, and inspection criteria.
