a. Memorandum from Provincial Director, Rizal PPO dated June 23,
2023 with subject same as above;
b. Memorandum from Chief, RIDMD dated June 23, 2023 with
subject same as above in relation to the Memorandum from this
office dated May 30, 2023, with subject: “Result of Initial
Evaluation against PCpl Rene Louie Olinares San Diego, BN
183683 of PARMU-Rizal PPO re Grave Misconduct (Violation of
RA 9262- Physical Abuse)

2. This pertains to the above-mentioned case wherein this station was

directed to personally coordinate and advise Ms. Lorna Olla Labay, 43 years old,
born on July 28, 1979, native of Mindoro, presently residing at Lot 3 Block 6, Phase
4A, Eastwood Greenview, Brgy. San Isidro, Rodriguez, Rizal to appear before the
Pre-Charge Evaluation and Investigation Section, Office of the Regional
Investigation and Detective Management Division (PCEIS-ORIDMD, PRO4A) on
June 26, 2023 (Monday) for some clarification on the above physical abuse case.

3. In this connection, please be informed that at about 10:40 AM of June 24,

2023 Investigation personnel led by PCPT UMAR M ADUCA, Chief Investigation
Section personally went to the said address and coordinate with Ms. Labay for her
scheduled appearance before the office of PCEIS-ORIDMD, PRO4A. Thus, this
office informed the complainant that we will assist and provide a transportation to
and from her residence on schedule date and venue, however she declined and
assert that she will appear using her personal vehicle. (See attach photos)

This office coordinate with Ms. Labay for her scheduled appearance before the office of
PCEIS-ORIDMD, PRO4A. Thus, this office informed the complainant that we will assist and
provide a transportation to and from her residence on schedule date and venue, however she
declined and assert that she will appear using her personal vehicle
