Good day! I’m Karen A. Eva a BsCrim Student, Last December 12, 2023, I didn’t able to attend my
class in Cybercrime at the time of 1:00 to 02:00 p.m. and that day may classmate inform me that I get an
FDA (Failure Due Absences) in short, I exceeded seven absences in this subject. So, I immediately write a
later to my Professor, Fiscal Alessandro Jurado stating the reason why I’m absent and he told me to
explain it to you and bring some documents that shows the reasons with my absences.

Last November 21, 2023 My teacher in P.E 3 Mr. Argie Hompay inform us that we are having a Water
Survival in November 23, 2023 at Greenfields 1 GIHA COURT from 08:00 am to 04:00 pm, but we finish
the swimming class at almost 04:00 p.m. so I didn’t able to attend the class that day.

And in December 14, 2024 at 11 a.m we are having are final defense and we finished it around 12:30
p.m.

December 07. 2023 ng naaksidenti ako while doing CAYGO, nadulas ako at nadaganan ko ung braso ko
and that day di ko magalaw ung braso ko dahil sa subrang sakit , I wasn't able to see a doctor because I
don't have a money that time because I already send my salary in province because my sister in the
hospital delivering her first baby.

I really sorry for this, I realized that it's easy to to attend my class that to make this kind of later, I
learned that there are somethings that I can’t predict that will happened, that’s why I ask for a second
chance I'll promise that I will make an improvement not only in my schedule but also by prioritizing my
studies.
