BIOGRAPHICAL SKETCH
The author, ADRIEL JAMES B. BAISAS was born on September 1,
2004. He is the middle child of Mrs. Josephine Vittone B. Baisas and Michael
A Baisas.. He attained his kindergarten and elementary years at Quinale
Elementary School and graduated with academic excellence. He then spent
his Junior High school years at Poten and Eliseo MQMNHS and graduated
with an academic excellence award. He is now pursuing his Senior High
school years at Paete Science and Business Colleges and will be graduating
this school year.
When he was young, he was aspiring to be an Exceptional Basketball
Player, he always joined summer leagues and tournaments.
The author believes that he will finish this course because he has an
inspiration and that is his family, who are very supportive, helping and
sacrificing for him to have a successful career. The author is also thankful to
God, our Creator, because He gave him wonderful and good parents.

EXECUTIVE SUMMARY
I, ADRIEL JAMES B. BAISAS, a STEM student from PAETE
SCIENCE AND BUSINESS COLLEGES, had completed a total of one
hundred twenty (120) hours of work immersion in BARANGAY 3 ERMITA
PAETE, LAGUNA from March 15 – April 3, 2023 On-The-Job Training or
OJT is job training that occurs in workplace. It is part of the Senior high
school curriculum where you are able to train your skills about the work and
your future career. It is very important especially to those students who want
to learn about their future job and to show students the reality of work.
In the span of Four Weeks, I was able to show my skills, I was able
to improve my own skills and knowledge, and I was able to experience
working my future job. It is a big challenge for me to interact with people in a
new environment especially for my young age working like a regular
employee.

Chapter 1
INTRODUCTION
Work immersion is a crucial aspect of vocational education and
training that enables students to gain real-world experience and workplace
readiness. Here are some of the key reasons why work immersion is
essential.
Work immersion programs allow students to gain hands-on
experience in their chosen field of study. This means that they can apply the
theoretical knowledge they have learned in the classroom to real-world
situations, helping them to better understand and retain the information.
Work immersion provides students with opportunities to explore
potential career paths and gain insights into the types of jobs and industries
they may be interested in pursuing in the future.
Work immersion enables students to connect with professionals in
their field and build valuable relationships with industry experts. These
connections can help students secure employment opportunities and gain
access to valuable mentorship and career guidance.
Work immersion programs help students to develop critical
employability skills such as communication, teamwork, problem-solving, and
time-management. These skills are highly valued by employers and can
increase the likelihood of securing job opportunities after graduation.

Historical Background of the Company
Ang brgy. 3 Ermita ay pangalawa sa maliit na brgy, ng bayan ng Paete,
lalawigan ng Laguna. Mayroon 3 Sitio nasasakupan; Sitio Sta. Ana., Sitio Sta.
Ana Tatlong Krus, at Sitio Bubon kung saan ang pinagkakakitaan ng mga
mamamayan ay sa pamamagitan ng pagkakaingin. Sa Sitio Sta. Ana Tatlong
Krus ay matatagpuan ang isa sa tourist attraction ng bayan ng Paete, ito ay
ang “Tatlong Krus” kung saan nakatayo ang 3 sementong krus sa tutok ng
bundok, may magandang tanawin at preskong hangin kaharap ang Laguna
de bay at ang buong bayan ng Paete, Laguna at dinarayo ito sa panahon ng
bakasyon at Mahal na araw ng mga lokal at dayuhang turista. Ang nasabing
brgy. Ermita (bayan proper) ay matatagpuan naman sa gitnang bayan ng
Paete; kung saan mayroon munting Kapilya, at mayroon siyam na kalye.
Ipagpalagay natin sa maikling pagtukoy ang kabihasnan noon ay sadyang
“bubot” pa, alalaon sa baga mga bungang-prutas; at walang mag-aaklang
sasapi t ang panahonng mahinog katanggap para makain ng tao. Hindi pa rin
“sayad sa laylayan”ang pag-usad ng sibilisasyon, dahil noo’y nasa
kalaginaanpa tayo sa “panahon bato”. Sino ang maaaring manghinuha na
itong bayan ng Paete sa ngayon ay maging durungawan ng Sining at
Industriya? Ating pagmasdan ang kapaligiran nitong baya; ang galaw ng mga
tao; ang sikad ng pag-unlad sa tahanang mamarangya; iba’t-ibang sasakyang
pangluypa, parianamit, pagkaing pisikal, kagamitan/kasangkapang pang
araw-araw, at marami pang iba. Sadyang nasa bulwagan tayo ng tiniklop na
telon ng kabihasnan. Sinasabi ata ting nakikita sa matatandang larawan
niluma na nang panahon; na itong bayan natin ay totoong mabato-

naglalakihang batong buhay, nagkalat kahit saang dako. Bukod sito mayroon
pa rin mga higanteng punong kahoy ang nagpapaligsahan sa malalabayna
sangang taglay ang luntiang dahon sa kagubatan. Landas pa nnon ang
lansanangang Mayor, nagsisimula sa gawing timog, tumutumbok sa pahilga,
lisa pa ang lansangang ito sa tawag na “Kalungosan”. Sa pinakagitnang
bahagi nitong lansangan/landas, mayroong dalawang higanteng batong-
buhay, dito sa pag-itan, makikitang wari’y isang bibig na naka-nga-nga. Ito ay
silbing pintong pinapasukan sa tahanang kuweba ni Dumayan, ang mabuting
bandido tumutugis sa mga taong nangungulimbat, nandarambong sa
maraming “Malayo” na dumadating sa larangan ng pangangalakal.
Nagbubuhat ito sa bansang Tsina at Indonesya, sila ang pinuprotektahan ni
Dumayan maagap nitong natutugis at naihahapay ang sinomang panghas na
mang himasok. Isang araw, biglang bumuhos ang malakas na ulang padpad
ng habagat, kasabay nito’y (sa bukana ng landas) umaatikabo ang inagy ay
hiyawang gumambala dito kay Dumayan. “Bah! Mga Muros, dumadating”.
Pagkakataon ko na naman, tiyak na sasamantalahin nilang kumanlong sa
aking kuwebal”. Ang sariling kutob nitong ating magiting na bandido. Ilang
hakbang nalang ang agwat niya sa kaawayna palapit, at lumagay siyang
patagilid na kasubok sa tabi ng pinapasukang pinto, walang anu-ano’y
sinimulang tugising isa-isa ang lumapit na kaaway-bunot ng sibat – tugis,
Kung hindi sumasalisi ang mga mangungulimbat dito disin-sana’y laging
maayos matiwasay ang mga katutubo. Maligaya silang namumuhay sa
palakad ng kanilang Punong- balangay, anupa’t matagal ang pananatili dito

ngayon nga ay Paete; binagtas nito ang Paete, Lumbang at pumalacob sa
Paracle, Peninsula ng Bikol. Narating niya ang Pulo ng Filipinas taong 1567,
narahuyo /nabihag sa pangingibig sa dalagang katutubo/tagalog si Dayang-
Dayang Candarapa, itong si Juan de Salcedo ay makisig, matapang na
heneral buhat sa Espanya samantalang si Candarapa ang pamangkin ni
Raha Lankandula ng Tondo. Lumipas pa ang matagal na panahon bago
napayakap sa pananampalatayang tinaglay nitong mga Kastila, matagal-tagal
din silang pagano, ang mga kaanak ng magkapatid na Kando at Larag, sa
kanilang pagsama/pakikiisa sa bagong kapangyarihang banyaga(Kastila)
itong kanilang pook na natatayuan nitong batong-kuweba, ay pinabangunan
ng isang “ERMITA” wikang kastila na nangangahulugang “Bahay ng
Ermitanyo”, hanggang sa makabagong panahon ito sa ngayon nakadambana
dito ang banal na ERMITAnyo na si San Antonio Abad. Sa panahon ng
panunungkulan ng dating Alkade ng bayan ng Paete; si G. Manuel N. Agbada
Sr. taong 1956, ang Conceho Municipal ay naghain ng panukalang
sinangayunan nitong naturang Alkade; ang pagbahagi ng siyam na Puori o
Barangay sa bayan ng Paete, Laguna.

Vision
Ang Barangay 3 Ermita at ang mga mamamayan nito ang
magiging kaagapay sa mga pangarap na maitaas at makilala sa antas ng
lipunang ginagalawan hindi lamang sa pangalan, Ito’y maipagmamalaki sa
kanyang katangiang may pagkakaisa at kapit-bisig sa pagpapaunlad ng
kabuhayan, pagmamahal sa kalikasan, pagmamalasakit sa kapwa,
pagtalima sa batas, mayamang kultura at kalusugan, anyo ng pamayanang
kinikilala sa pulitikal, sosyal, Pisikal, edukasyonal at Espiritual.
Mission
Hawak kamay sa pagkilos sa pagkakaisa para sa mga adhikaing
kaunlaran at kasaganaan na nauukol sa kabuhayan, Paglinang sa kalinisan at
kalusugan, paggalang sa inang kalikasan, katahimikan at kaayusan, disiplina
at pag tuon ng pansin sa dangal at moral ng mga mamamayan, pantay na
paglilinkod at pag tugon sa mga suliraning kakaharapin ng mga
pinagkatiwalaang namumuno sa barangay, na may buong pusong pag
samba, pagkilala at pagtitiwala sa DIYOS na lumikha.

All of the Department in BARANGAY 3 ERMITA are dedicated to providing
and serving reliable services. The Barangay 3 Ermita Operates 8am-5pm a
day, 5 days a week to provide timely and efficient services to the clients.

Personnel In-Charge and Designation
Capt. Floradel Grace A. Funtanilla is the Capt.
Of Barangay Tres Ermita in Paete, Laguna. She has excellent and good
leadership in her Barangay and she ensures that all of her people are
disciplined. She also reminds us about our activities if there was activity or
something.
Ms. Majalh Navarro is the secretary of barangay and she is the duty
on paperwork and the list of families in Barangay. She always reminds us
about our paper works and other activities and also, she always teaches us
about everything in Barangay.
Mrs. Nalie Bunyi is a BHW and designated to teach us how the clinic
works and checking blood pressure and also, she’s our guide in every duty we
do.

Inclusive Dates of Training
The trainee received the on-the-job training at the Barangay 3 Ermita.
Starting on March 15, 2023, and ending on April 3, 2023. It is composed of
120 hours or 15 working days not including Saturday and Sunday.

Chapter 2
DAILY ROUTINARY ACTIVITY
In this chapter, the student trainee presents the day-to-day
standardized actions or procedures that are followed regularly and often
repetitiously.

EXPERIENCE AND JOB DESCRIPTION
On our first day at work immersion, the captain
called a meeting around 9:00 AM and explained
March 15, 2023 to us what we would do every day and also told

us to study on how to check a person’s BP. At
first, it was difficult to get because we still don’t
know how to use the BP, as, we asked the BHW
for advice.
We started at 8:00 AM and we waited, and
around 8:30 to 9:00 AM we went around to do

(Thursday) BP in barangay 3 Ermita among the elderly who
wanted to get their BP. We had a companion
who was a BHW to support us in our work.
We got there at 8:00 AM, waited, and then
went around to do blood pressure checks on the

March 17, 2023 elderly residents in barangay 3 Ermita. We have
(Friday)
a BHW partner to help us with our task.
This Saturday we attended the barangay
assembly of barangay 3 Ermita, and we were
March 18, 2023 ordered to attend the barangay assembly. The

(Saturday)
mayor and vice mayor of Paete and councilors
were there, and we also helped in providing
snacks to those who attended the barangay
assembly.
Elders in the area had recently had their blood
pressure tested. They were just waiting for
directions. We proceeded to navigate the
neighborhood, making our way from one
residence to another in order to visit the elderly
individuals residing in the area. The captain had
March 20, 2023 prepared a feast of buns and filling before

bringing us Coke when we got back to the
barangay. We were supposed to do something
and help clean the barangay, according to the
captain’s instructions. Papa concluded by
expressing that it is understandable to
empathize and express concern when others
face unfortunate circumstances since our
presence signifies our support for them.
I sliced the spaghetti into manageable portions

and arranged them neatly for Miss Cleo's
daycare center, catering to the dietary
requirements of the students. Additionally, I
chopped ingredients such as garlic and onions

March 21, 2023 as per instructions. After completing these
(Tuesday)
tasks, I was instructed to prepare supper and
ensure that the new students were fed. They
enjoyed a meal of spaghetti, which was
prepared by Miss Josie. The pasta was served
to the children, as per Mr. Cleo's request, and
Miss Josie took care of feeding the kids.
We prepared macaroni once again, and I took
on the task of slicing and peeling the haloks. In
addition, we cooked a stew for the feeding
session. During this time, we became aware of
visitors from another school who had come for
immersion in our barangay. We quickly engaged
in conversation with them, forming an instant

(Wednesday) bond. They offered us an opportunity to
collaborate on a project, and they also assisted
with feeding us once the children had finished
their meals.
Upon entering the building at 1:00 pm, we
noticed the presence of visitors. The secretary
was speaking, and the captain informed us,

"Alright, you are waiting for me to take your
blood pressure." There were three elderly
citizens in the vicinity, accompanied by the
visitors. At 3:00 pm, the visitors departed from
the premises.
I prepared a delicious tuna and vegetable
omelette which received great feedback once it
was cooked. I actively participated in various
duties here, including cutting meat, tofu, and
vegetables. I also opened and separated the
tuna for the soup and cracked open the eggs.
To help me work more efficiently while the
women in the barangay cooked, I kindly

(Thursday) requested assistance.
Upon our return, we found that the omelette had
already been cooked, so we swiftly prepared it
for the kids' lunchtime. The children then had
rice along with their omelette, which we served
on plates. Some of the kids insisted on taking
their food home, so we packed it for them. As
for the legumes, once they finished cooking, we
used the kids' food containers to store and
distribute them.
We checked our blood pressure in the morning
before returning to J’s house. There was a lot of

blood pressure on Luna Street. We were taken
by the gallons to the barangay when we got
back so the kids could wash their hands. We,
the kids, were the ones who were sent in right
away after the daycare students’ break since
they had a class they were instructing and
teacher Cleo was watching cartoons before we
went home. Children of ours reside in a nearby
barrio. We took them to our home and then left.

March 24, 2023
The captain gave us civil service when we got
(Friday)
back. He said that maybe because our clothes
would be painted at 1:00 p.m., we gave them to
him. I have a job. The captain gave me a no-
parking poster and a pencil bag. We painted on
Pateros Street because there is no parking.
There is a line and a painter along with me. I did
the painting for two of us, and we did it for two.
When I was writing around 3:00 pm, the captain
called us to have a snack in the barangay.
When we finished having a snack, we
immediately went back to our work. There were
five of us, no parking on the street of the village,
and we finished at 5:00 pm. Then we got home.
I woke up early with the intention of visiting the
barangay. Since we were about to leave, we

wanted to ensure that both the barangay
premises and its surroundings were clean.
Therefore, we began by cleaning the exterior
areas. We then gathered at the location of the
three crosses, allowing everyone to be present
for the congregation. Apart from the captain, the
rest of us traveled on motorcycles, while some
of us chose to walk. Despite the exhaustion, we
persevered and continued our ascent towards

the top of the three crosses. This tradition holds
(Saturday)
significance during the Holy Week at the
hermitage.
To maintain cleanliness, we proceeded to the
designated no-smoking tent area, emphasizing
the importance of not littering. Later, at
approximately nine in the morning, we had our
meal. Following the meal, we measured the land
for the shops, conducted barreta (clearing and
leveling off the ground), and set up the shops
accordingly.
During the Holy Week, as we tied the trees, we
assigned numbers to each tree as they held
individual significance. Around 11:30, my
colleagues and I concluded the work immersion

and made our way back home
On the eleventh day in the barangay, I started
my day by checking blood pressure. Observing
that the others seemed exhausted from our
morning work, I decided to lend a hand in
preparing supper for the kids. In the afternoon,
we received a lecture from the captain as we
March 27, 2023 were tasked with painting humps. The purpose

of painting the humps was to ensure that
motorcyclists could easily navigate them, while
pedestrians could appreciate the artistic
drawings. As the location was on another hill, I
focused on completing a few humps before it
got late, around 4:00 pm. Afterward, the captain
and I took a break to have a snack. Once we
finished our snack, we were dismissed and sent
home at 5:00 pm,with the understanding that the
work would continue the following day .
Today, my morning started by conducting blood
pressure measurements for a few individuals.
Alongside Jericho, we proceeded to visit
household to check the blood pressure of those
who preferred not to go to the barangay.
Kapitana provided us with instructions regarding

(Tuesday) painting the streets tomorrow afternoon. We

commenced painting the remaining half of the
humps that we had left unfinished from the
previous day, which resulted in a visually
pleasing outcome.
We arrived at 8:00 am, and the captain told us
March 29, 2023 to go to the nativity school in barangay 6 so we

could get a signature from the principal on
behalf of the pupils there.
Today, I started my morning by engaging in
painting activities along the street.
Subsequently, my companions and I embarked
on a neighborhood tour to conduct blood
pressure checks and provide assistance in meal

(Thursday) preparation for the children. Once we finished
painting humps and "no parking" signs on
Dalagan Street, our next task will be to paint
Barangay Ermita, thereby completing the entire
street painting project in Ermita. In the
afternoon, we will resume our painting activities.
On my final day of work immersion in the
barangay, a police officer visited and provided
April 3, 2023 us with important information about Republic Act

8353, commonly known as the "Anti-Rape Law
of 1997." The officer emphasized the
significance of understanding and preventing

rape, offering guidance on how to avoid such
incidents.

Chapter 3
SELF-ASSESSMENT
In this chapter, the student trainee comprehensively self-assessed the
additional knowledge and skills achieved during the training program.
Achievements
a. Skills Learned/Enforced
One of my most memorable experiences during my time in the
barangay was participating in the blood drive at the three crosses and
conducting surveys among the community members. It was a
rewarding and impactful opportunity to contribute to the well-being of
others and gather valuable information to better understand their
needs and perspectives.
b. Equipment, Machinery, Testing Apparatus, etc.Handled
In the barangay, various machines and tools can be found that
serve different purposes, although I may not be familiar with their
specific names. These instruments are primarily used for blood-related
procedures, such as blood tests and diagnostics. They aid in detecting
any potential issues through the use of advanced technology.
Additionally, there are supplies such as blood vials and syringes
dedicated to blood-related procedures. For general health
assessments, equipment such as blood pressure monitors,

thermometers, weighing scales, and measuring devices are available.
Furthermore, there is a computer present in the administrative office,

serve administrative and operational functions, facilitating tasks and
information management within the barangay.
c. Strong Points Versus Weakness Point
The trainees have the ability to cope with any adjustment and
he is punctual and creative, but his weakness that affects him is if
there's no one willing to support him.
d. Best Experience on the Job
One of my most rewarding experiences was participating in the
blood drive at the three crosses and conducting surveys among the
community members.

RECOMMENDATIONS
All kids who want to learn about computers can take several lessons
in the barrio of Tres Ermita, which serves as a good training ground for them.
We are pleased to be the first to train in the hermitage since we were the first
to do so. Here in the barangay, they can study the safety regulations and
how to ensure safety in the event of an emergency.
a. Availability and Appropriateness of Training, Facilities, Equipment,
Tools and Machinery
The barangay should expand their office so that they can act properly
and not be crowded.
b. Company Cooperation Personnel
All the staff within the barangay are exceptionally kind and have a
great demeanor particularly Ms. Majalh Navarro, the secretary of the
barangay a.k.a Sec Mahj we are exceptionally fortunate since she
always gives us lessons to memorize and gives us exhortation for
long haul and appears us all what we’ll do on the off chance that we
are a successful person within the future.

Advised to Future Student-Trainee
My exhortation to understudies who need to do OJT here within the
barangay I would like to propose that usually the correct one if you get
experience this can be the great barangay for you. Since all the staff here
will watch out for you and they will grant you a lesson for your trip, you may
realize something here within the barangay, almost how to communicate
with each other within the future, you’ll learn a parcel here. Within the
barangay is the computer course. Continuously teach for yourself and have
a great state of mind towards all the staff here. It’s not a terrible life, it’s a
fair, awful day you’ll continuously keep in mind.
