AFFIDAVIT

I, SAGAYA MARY daughter of JOSEPH wife of S STALIN Residing at

No. 1253, Block-33, BDA flats, Austin Town, Bangalore-560047 do hereby
solemnly affirm and state on oath as follows,

I declare that I was student in Government High school, Paper Town,

Bhadravathi, and I have completed my SSLC in the year 07-12-1983 and later
concerned authorities issued my transfer certificate.

I further declare that I have my original transfer certificate while travelling in my

residence surrounding area on 20-11-2017 which comes under viveknagar police
station limits and which could not traced elsewhere.

I request to the concerned viveknagar police station authorities to register the

complaint and give me acknowledgment

This affidavit is be produced before the viveknagar police station authorities to

register the complaint for obtaining above said transfer certificate for my job
purpose

If in future date traced elsewhere I will surrender to the same to concerned

authorities
