and state on oath as follows.

I declare that I am residing at above mentioned address.

I further declare that I am legally wedded wife of late. RAMASWAMY and he

expired on 12-05-1990 at Bangalore

I further declare that I was working as a office maid servant in Air force School,
Murugesh Palya, Bangalore

I further declare that I have no documentary proof to produce my husband

death certificate and I have retired from the service

I further declare that I do not possess any joint photo my self and my husband
self

I further declare that I affixed my photo here

This affidavit is to be produced before the Air force School, Murugesh Palya,
Bangalore authorities to confirm the above facts.
