AFFIDAVIT UNDER OATH

I, Md Rakib Pervez (BANGLADESHI Citizen holding BANGLADESHI Passport No. A02806334)

residing at Sadhu pandit bari,south salimpur,sitakunda,jafrabad,chittagram BANGLADESH,
do hereby solemnly taking oath and states as below:

• I vow I'll be back in BANGLADESH on NOVEMBER 29, 2024, when my contract ends. I
won't remain in SPAIN any longer than is absolutely required, so neither the country
nor the state will have to support me.

• I hereby confirm that I have previously received from my employer companyan

authenticated work permit, a guarantee letter, an accommodation letter, and other
required documents that support my employment and my right to remain in Spain
until the conclusion of the contract. I hereby also declare that I have never been
accused of committing a crime in Bangladesh or been judged to be guilty of one.

• I've also provided the schedule for my reserved return flight to BANGLADESH, which
I'll book once my visa application is accepted.

• I hereby declare that my entire family resides in BANGLADESH. I'll return to

Bangladesh after my employment contract expires to live with my family because I'm
close to them and we live happy lives here. I am also responsible for their well-being.

I confirm that the information above is correct to the best of my knowledge and belief. If the
information turns out to be wrong, I am aware that I may face consequences.
