    AFFIDAVIT

    I, GALLA REPUBLIC RANI, W/o. G. VENKATESWARA RAO, R/o.D.No.4/157,


    Surampalli (V), Gannavaram (M), Krishna Dist-521212, do hereby solemnly affirm &
    state on oath as follows:-

    I hereby declare that I am the owner of the house bearing of D.No. 4/157, Surampalli (V),
    Gannavaram (M), Krishna Dist-521212.

    I further submit that the One room at First Floor in my house is given at free of rent for
    the location of society which I am the president under the Name & Style of “ AMMA
    PREMASHRAM” in the interest of the Society. I am not collecting either directly or
    indirectly any rent from the said Society. Hence the Society may be registered.

    I declare that the particulars stated above are true and nothing has been concealed or
    hidden therein.
