I, KRISTIAN PAUL DIZON ENORE, Filipino, of legal age, and with

address at B11 L5 Daisy St., Adelina 3, Phase 3, Santo Tomas (Calabuso), Biñan
City, Laguna, do hereby state that:

1. In June 2019, I enrolled at Polytechnic University Of the Philippines

Biñan Laguna Campus, with address at Brgy. Zapote, Biñan City,
Laguna;

2. On 13 October 2023, I graduated from the same school with a degree of

Bachelor of Science in Information Technology;

3. From the time I enrolled, until I graduated, I attest that I was never
issued with a school identification card for unknown reason/s;

4. Thus, I am executing this affidavit to attest to the foregoing facts and for
the purpose of obtaining a copy of my Transcript of Records from the
school, and whatever legal purpose this may serve.
