THIS IS TO AUTHORIZE MS. AUXILIADORA L. ALVAREZ, Medical Specialist III

at Salvacion Oppus-Yñiguez Momorial Provincial Hospital, Dongon, Maasin City, Southern
Leyte, to travel to London on May 5 to June 4, 2018.

This permit is issued provided that no funds from the Provincial Government of Southern
Leyte shall be spent relative to the said travel and that all necessary expenses shall be the
personal responsibility of the above-mentioned personnel.
