1. Introduction
This document is developed as a guideline of Contractor and its Sub-Contractor employees who are
involved in road transportation activities in order to prevent traffic incident.
This document will be applied to RDMP Project Balikpapan area are either in brown area or green
area that shall be controlled by Contractor.

2. Purpose
The purpose of this is to define the minimum acceptable standard for land transportation activities
within Contractor operations. This document is provided to all management with a guideline for
implementing the standard of HSE Management System and safe working practices in order to
minimize risk or to prevent potential hazards, which may affect to personnel or to prevent damage of
equipment/assets or environment.

3. Scope
This document applies to all road vehicle used by Company, Contractors, Subcontractors and visitors
on the field who has activities, where Contractor has prevailing HSE influence.
This document applies to all vehicles and mobile plant involved in road transportation except forklift,
cranes and other specialist used within plant sites and site support areas.

Defensive Driving : Driving to reduce the risk of collision by anticipating dangerous situations,
despite adverse conditions or the mistakes of others. This can be achieved
through adherence to a variety of general guidelines, such as following
the assured clear distance ahead and two second rules, as well as the
practice of specific driving techniques
Driver Competency : Demonstrated knowledge and skill required by the attached Contractor
Driver Competency Standard
Field Road : A field is any road or track within a Contractor area of operations as
defined by Contractor Management. A field road may be a public or
private road.
Field Vehicle : A field vehicle is any leased, hired, Contractor owned or
Sub-Contractor Owned for using on field roads. Including trucks,
MPV, SUV and other vehicles designed (Crane trucks) but excludes
bicycles, heavy equipment (crane, excavator, forklift)
Heavy Vehicle : A vehicle with gross mass of greater than 4.5 tones
Heavy Equipment : Heavy-duty vehicles, specially designed for executing construction tasks,
most frequently ones involving earthwork operations.
IMKDK/SIMPER : Ijin Mengemudikan Kendaraan Dalam Kilang
ID card of registration and identification issued by HSE RDMP
Balikpapan to driver who have met the requirements for driving a
vehicle. HSE RDMP Balikpapan will be tested the driver before issued
the IMKDK/SIMPER.
Light Vehicle : A vehicle with gross mass of less than 4.5 tones

Vehicle Accident Rate : Vehicle accident rate is number of vehicle accident per million
kilometers driven (for light vehicle) or running hours
(for heavy equipment). Calculation (Vehicle accident case /
Total kilometers driven or running hours) x 1.000.000

6. Procedure
6.1 Performance Monitoring

To achieve project goals in HSE and minimize transportation and journey risks associated in the
project activities, Contractor set performance standards with the aim of eliminating and controlling
risks. The ultimate project specific transportation performance target for this project in table below
named Transportation KPI’s Performance Target:

6.2 Duties & Responsibilities

a) Dispatcher

• Shall ensure the maintenance program of vehicle are implemented at the project.
• Shall ensure the mobilization of vehicle as per requisition of user.
• Shall maintain consumable material stock in order to continue support project.
• Shall ensure the operators and the drivers of project vehicle are competent and certified
person.

b) HSE Manager

• Shall ensure the driving policy is socialized to all employees and Sub-Contractor
employees.
• Shall ensure that all subcontractor vehicles are inspected prior enter to the project site.
• Shall arrange HSE inspector to conduct vehicles inspection in regular basis.
• Shall manage the installation of traffic signs at surrounding of project area and/or
supported area of project (warehouse, jetty, and workshop).
• Shall ensure all project transport obeying public traffic once driving at the public area.

c) Chief HSE

• Has responsible to report to HSE Manager about vehicle inspection results


• Shall ensure registration system of new vehicle that will be used in project site.
• Shall ensure all operated vehicles at project area are safe manner and all drivers or
operators area aware with safe driving rules.
• Shall ensure all driving procedure implemented accordingly.

d) HSE Inspector

• Shall assist HSE Manager to inspect any new of vehicle prior operating at project site.
• Shall carry out regular inspection for all vehicles that operating in the project site.
• Has responsible for determining the route that is safe to pass.
• Has responsible for carrying out escorts for vehicles that not have pass permit sticker.
• Shall checking randomly the passenger’s seatbelt, the passengers only equal with seatbelt
provided.

e) Supervisor

• Shall ensure the validity of vehicle pass permit sticker and operator’s IMKDK.
• Has responsible for carrying out escorts for vehicles that not have pass permit sticker.
• Supervisor shall assist to HSE Manager to socialize driving policy to all driver and
operator in the project area.

f) Driver

• Driver shall hold valid driving license for the type of vehicle that they are operating
within project site and valid IMKDK/SIMPER.
• Driver shall obtain safety driving course which conducted by HSE Dept. and refreshed
annually.
• Driver shall obey company driving rules and respect with others warning signs posted.
• Driver shall conducted a daily inspection/pre-start check list of the vehicle that will be
used in project are (see Appendix-1).

• Driver shall immediate report to the user if any defect finding during perform a daily
checklist.
• Driver is responsible to report if any incident in order to their vehicle that operated by
their own.
• Driver shall ensure that STNK/vehicle register is valid, he shall report to the user about
the validity of the vehicle license if necessary.
• Driver is prohibited from using mobile phone or two way communication devices during
driving.
• Driver is not under the influence of drugs, alcohol or others substances that can
undermine their driving capability.
• Ensure are lashing/ choke/ stanchion has been installed properly.

g) Vehicle Requirements

• All JO’s member owned/leased vehicles shall meet with JO’s HSE standards
• All JO’s member owned/leased vehicles shall complete with valid car certificate (BPKB)
and car register number (STNK) issued by police.
• All JO’s member owned/leased vehicles utilized for transporting material or equipment in
project area shall obtained an inspection by HSE Dept.
• The requirements for vehicles age that are permitted to operate and have passed
inspection from safety aspects are as follows :

6.3 Safety Driving Rules

1. Ensure all vehicle that will be utilized in good condition and equipped with appropriate hand
tools, fire extinguisher, flash light, warning triangle and first aid kit.
2. Before getting ready for a trip, make sure the driver was in good health and alert.
3. Driver and all passengers should always wear and fasten seat belt and ensure that the seat
belts are in a good condition and function well.

4. Using phone or other two way communication devices while driving is forbidden.
5. No open cabin allow to drop or pick up passenger for overall project area.
6. Keep safe distance with front vehicle.
7. Push the horn and switch on the signal lamp while overtaking another vehicle
8. Never overtaking from the left side, blind spot area, bent of road and uphill road.
9. Reduce the speeding or turn to the left road while passing with the emergency vehicle.
10. Parking only allowed around designated parking area.
11. Heavy equipment shall be equipped with rotary light and always on all time.

13. For night driving, ensure vehicle’s main lamp, rotary lamp (for heavy vehicle) are
functionally properly.
14. Driving after consuming alcohol, using mobile phone (phone or texting) while driving
around project will be dismiss from project instantly.
15. Each violation will be given disciplinary action refer to 26071-100-GPP-GHX-00029 –
Rewards and Punishment Procedure.
16. All vehicle and truck only allowed to park at designated parking area.
17. All vehicle’s muffler must be equipped with flame arrestor.

6.4 IMKDK/SIMPER

Driver shall be assessed driving qualification organized by HSE Dept. HSE Dept. conducts
IMKDK/SIMPER issuance process, written theoretical and practical assessment on road
(commentary drive) as a requirement for driving vehicles. The requirement for issuing
IMKDK/SIMPER as follows:

1. Submitting a copy of valid ID Badge, defensive driving certificate and driving licenses
(SIM).
2. Carrying recommendation letter from Contractor or Subcontractor.
3. Passing a written theoretical test & practical test conducted by HSE Dept.
4. IMKDK/SIMPER is valid in line with contract duration or SIM.

The type of driving license from Indonesian Government


SIM A : To drive a public motorized vehicle and items with the allowed amount of weight
do not exceed 3,500 kg.
SIM B1/ : To drive passenger cars and general materials with a total weight of more than
B1 Umum 3,500 kg.
SIM B2/ : To drive a towing vehicle or motorized vehicle by pulling a train or trailer with the
B2 Umum weight allowed for a train or trailer with more than 1,000 kg.

IMKDK/SIMPER design

6.5 Rest Break Arrangement

• Each driver shall have enough rest when operating vehicles in order to keep alert and they shall
not operate vehicles when they are tired or fatigued.

• Overtime authorization mechanism to drivers (if necessary), is implemented with the approval
of the authorized supervisor.

• The supervisor shall consider working hours and rest time of the relevant driver before giving
authorization for over time.

• Each driver shall stop driving when tired or fatigued. Journey shall be arranged in such a way so
that driver shall have enough rest.

• Minimum requirements for working hours and driving are following:

6.6 Loading/Handling Material and Equipment

• Before entry main gate project area, the driver shall be reported to the security post.
• Contractor and Subcontractor shall inform and ensure all vehicles are inspected by HSE Dept
prior enter to Project area.
• Material and equipment shall be loaded or unloaded properly and stacked firmly to avoid
shifting/moving during mobilization (e.g. land transport) and all material and equipment shall
be secured.
• Truck tire shall be choked while transferring material (loading/unloading).
• Prohibited to transferring material longer than the flat bed unless it has obtained permission
from HSE Dept. based on material load calculation, the maximum allowable length is not
more than 1/3 of the flat bed total length.
• Lashing system must be provided as additional safety system during transported material and
equipment.
• To prevent material rolled out especially for tube material, pipe, tube pile shall be choked and
choked with stanchion.
• Before leaving the loading/ unloading bay, operator has responsibility to check and ensure are
lashing/ choke/ stanchion has been installed properly
• When transporting the material or equipment are more wide that the cabin or bulk, an
escorting system by HSE Dept. must be provided to clear the access.
• When transporting heavy load the driver shall clearly and familiar with the route, road bearing
capacity, bridge capacity, overhead power cable and has permitted by police or other

authorities.
• Vehicle designated for hazardous material transportation shall comply with applied regulation
such as government's permit for hazardous material handling.
• When stopped on machines, driver shall ensure that parking brake is working properly and put
the wheel stopper (especially in elevation position).
• Do not open lashing unless the truck already on position for unloading.
• During unloading process driver/ operator are not allowed to stay inside the cabin unless based
of special requirement stated the operator shall be inside the cabin.

6.7 Journey Management

Contractor shall have an effective Journey Management System that will be addressed the risk
associated with the driving activities. All vehicle are operated in the project area shall be managed by
the Journey Management System. The Journey Management System shall be updated about the
layout or the route that be used as guideline of the track of vehicle and shall take into account any
possible emergencies that may occur including injury to personnel and property/asset damage and
environmental damage (from vehicle loads etc). As a minimum the system must include the
following elements are:
• Identification of hazards, assessment of the risks identified and implementation of appropriate
control to reduce risks to safety and the environment associated with journeys to as low as
reasonably practicable (ALARP).
• Requirements for reporting and recording vehicle movements.
• Communication arrangements.
• Action to be taken by personnel in vehicle in the event of an incident during the journey.
• A site or area response plan to detail :
- Procedures to be followed when an incident occurred and reported.
- Responsibilities for monitoring vehicle journeys and determining immediate and follow
up actions under the system.
Special consideration should be given to new or inexperience personnel prior to driving. Those
personnel shall receive a detailed briefing about driving conditions and hazards specific to their area
of operation.

Vehicles shall be cleared when moving from one region to another, or from a known weed infested
area to prevent spread of weeds.

6.8 Vehicle Operations

Vehicles limitation allowed operating in brown field, with an additional identity for the vehicle in the
form of yellow scot light in the right and left part of the vehicle body. At no time shall a driver of a
moving vehicle operate a mobile phone or other two way communication device. This is a regardless
of whether or not the vehicle is fitted with a “Hands Free” device. Drivers are required to pull over
and stop in a safe location (allowed area) prior to making or receiving mobile phone calls.
When driving in the field there are a number of existing and additional rules that need to be
highlighted :
• No driver shall exceed government prescribed speed limits or Contractor management
imposed speed limits, which ever is the lesser.
• Contractor management may declare any road closed or limit its used by Contractor and
Subcontractor personnel for safety or operational reasons.
• Vehicle shall be suitable for their intended use and be regularly serviced to maintain their
safety and reliability.
• All drivers must carry a suitable map of the area in which they are going to be travelling when
driving in unfamiliar territory.
• Driver shall not drive or overtake when vision is impaired (e.g dust, rain, smoke, insect, etc).
• No person shall operate equipment while at an elevated risk of unwanted events arising from
the use of alcohol and other drugs. All drivers shall be obtaining the random testing of alcohol
and drugs from paramedic.
• Seat belt shall be fitted to all vehicles used by Contractor and Subcontractor operations.

6.9 Tires and Rims

Maintaining tires and rims are a routine task than quickly become a high risk activity with potential
fatal consequences unless the precaution are taken. There are four main hazards with tires and rims
are:
• Vehicle falling off the jack or support while removing wheel due to unstable ground,

overextension of the support, the jack not being properly located or the vehicle not being
checked to prevent movement. To reduce the risk of the vehicle falling off the jack. The
vehicle shall be secured as follows before commencing to remove a wheel :
- Switch the vehicle off and remove the keys
- Place the vehicle in first gear
- Apply the hand break
- Chock at least one wheel diagonally opposite the wheel to be changed.
• Manual handling – due to the heavy and difficult shape of tires and rims.
• Pressurizing big tires shall use a cage to avoid tires ring loosing and hit the person around.
To control the risk associated with tires and rims the following requirements shall apply :
• Before any bolts are loosened to remove a wheel from a vehicle the tire shall be defeated (both
tire in a dual assembly)
• Appropriate facilities and equipment shall be provided for the safe removal and assembly of
tire and rims including :
- A guard or safety cage to prevent any injury to the tire if a failure does occur.
- A pressure limiting device such as pre-set air gauge on air hoses (don’t over inflate a tire
beyond the recommended inflation pressure stamped on the side on the tire unless
recommended by manufacturer).
• Only competent person shall remove and fit tire to spit and tubeless rims
• Tire pressure should be checked when the tire is cold and the pressure maintained between
manufacturer specification limit, typically 32-35 psi
• Tire shall be replaced in accordance with manufacture specification for wear and damage.

6.10 Care and Maintenance of Road Transport Equipment

As a minimum maintenance on the road transport equipment shall be conducted in accordance with
the mandatory maintenance specifications.
Pre-start and periodic inspection shall be conducted and documented by drivers or operators. The
frequency checking of vehicle shall be determine as commitment is taken by each department.

6.11 Heavy Vehicle

1. Heavy vehicle drivers shall hold valid driving license and competent to drive in project area.
2. Contractor heavy vehicle drivers shall keep personal log books for all travel conducted.
3. Drivers and passenger shall enter and exit vehicle using steps and hand rail provided and shall
not jump from the cabin or higher level steps to ground level.
