SCENARIO

XYZ fire equipment company is in to manufacturing and installation of fire detection, alarm and fighting
systems. They have a manufacturing plant and a warehouse. Manufacturing division consists of 150
workers, comprised of , a plant manager, production manager, maintenance manager, 5 administration
staff, 3 production planners, 3 production supervisors, 1 maintenance supervisor, 5 maintenance
technicians and rest are production technicians. Warehouse has a warehouse manager and 10 warehouse
operatives. The company has an installation department, which is comprised of a commissioning manager,
5 supervisors and 75 workers who visit client’s premises for installing the fire detection, alarm and fighting
systems. The company has a health and safety department with a manager and 5 practitioners who take
care of the health and safety systems and practices.

The company has a health and safety management system ( ISO45001:2018 ) implemented.

The company has a comprehensive Health and safety policy covering aims and objectives of the
organization, management commitment and is signed by the CEO of the company. The H&S policy is
communicated to the workers at the time of joining and it is also displayed on the notice board. Policy is
discussed during departmental meetings and included in the company intranet too.

Both the warehouse and production facilities are well maintained. All the equipment are well guarded and
regularly maintained, Regular housekeeping, adequate lighting, supply of water, air conditioned
environment, enough number of toilets, cloth changing facilities, dining room etc. are arranged. There is
proper segregation for the vehicle and pedestrian traffic. One way system and reverse parking is
encouraged in the facility

At the beginning of the year, department managers, HSE department, supervisors and employees meet
together and set the H&S KPIs. Every department has identified the hazards and risks involved in their
activities and risks are well controlled. Workers are consulted and encouraged to take part in risk
assessment.

The company has a good incident reporting and investigation system. Employees are trained how to use
the reporting system. The company ensures that all types of incidents are investigated and lessons learned
are shared with all the departments.

The company has an internal as well as an external audit conducted once in every year to ensure
compliance with the HSMS. The company has annual management review. It has a safety committee with
equal participation from workers and management. A suggestion scheme and rewarding scheme for the
safest employee is also in place.

The company has a proper recruitment policy and procedure and provides its employees induction, job
specific and refresher trainings. The supervisors are trained on supervisory skills. The senior managers
and middle level mangers have also finished H&S training.

You are a newly joined HSE supervisor for one of the new installation project. The current project you are
handling is a high risk project. The project involves installing the detection and alarm systems in atriums,
where the ceiling is at more than 12m height. It involves erecting a scaffold. Since the company has never
undertaken such complex installations, it has arranged for a competent consultant to assist with RA and
developing a safe system of work. Workers are also consulted and their concerns were discussed in an
open meeting.

A competent contractor is selected for the installation of scaffolding. A contractor co-ordinator is appointed.
The inherent hazards and risks of the job is communicated to the contractor and contractor is instructed to

submit their RA and method statement. The company has developed emergency rescue plan for fall from
height and conducted an emergency rescue drill too. All suitable equipment are arranged and proper
inspections are carried out. The company has had no accidents in the past 4 years and has good reputation.
