सर्व नागरिकाांना कळवर्ण्यात ये त आहे वक, कोवर्ड -१९ चा प्रादु र्ावर् कमी किणेसाठी केंद्र शासन आवण िाज्य
शासनाने जी वनयमार्ली सर्ावना लागू केली आहे ती लक्षात घेता सामावजक अांति ठे र्णे हे अवनर्ावय आहे . तसेच आधाि केंद्रामध्ये
होणाऱ्या नागरिकाांची गदी लक्षात घेता UIDAI प्रावधकिणाने ऑनलाईन अपॉइां टमेंट वसस्टिम चालू किण्यात आली आहे. जेथे
प्रत्येक नागरिक खाली नमूद केलेल्या वलांक र्ि जाऊन आधाि नोांदणी किणे र् अद्ययार्त किण्याकरिता ऑनलाईन अपॉइां टमेंट
घेऊ शकता ज्याने सध्याची परिस्टथथती लक्षात घेता सामावजक अांति दे खील िाखले जाईल आवण नागरिकाांची गैिसोय दे खील
होणाि नाही.

सदि ऑनलाईन अपॉइां टमेंट सेर्ा वह वर्नामूल्य असून त्याकरिता आपल्यास कुठल्याही प्रकािचा शुल्क द्यार्ा लागणाि
नाही. पिां तु UIDAI प्रावधकिणामार्वत आधाि अद्ययार्त किण्यासाठी जे शुल्क ठिवर्ण्यात आले आहेत र् दिपत्रकामध्ये नमूद
आहेत तो शुल्क मात्र आपल्याकडू न आकािला जाईल.

ऑनलाईन अपॉइां टमेंट घेण्याकरिता आपण कोणत्याही सायबि कॅर्ेद्वािे आधाि नोांदणीकरिता ऑनलाईन अपॉइां टमेंट
घेत असताना जि ते आपल्याकडू न कोणत्याही प्रकािचा शुल्क आकाित असतील ति त्यास वजल्हा प्रशासन र् एनिोलमेंट एजेन्सी
जबाबदाि िाहणाि नाहीत. तसेच आधाि केंद्रचालक जिी आपल्याकडू न सदि कामासाठी पैशाची मागणी कित असतील ति
त्वरित वह बाब आपल्या सांबांवधत वजल्हा प्रशासन याांच्या वनदशवनास आणून द्यार्ीत.

I. Non-Document Checklist:

Name:

 Should not have any Salutations like Mr., Mrs., Dr., Col. , Adv., etc.

 Should not contain multiple names like Devi urf choti , Devi ( choti) etc.

 First, Middle & Last Name should exactly match as per the document
submitted by resident.

 Transliteration of Full Name in local language should compulsory match


as mentioned in English.

Address:

 The document (Electricity, Water, Telephone & Gas bill) should be on the
name of applicant. These documents on the name of father are not
acceptable for enrolment/update of son etc.

 Main address should exactly match with POA document submitted by


resident. Any additional details apart from the main address can be
added.

 Address shall be complete in all respect for ease of delivery.

 Try to make best use of “Landmark” field in the address.

 Translation of resident’s address in local language should match as


mentioned in English.

Photograph:

 Face should be clearly visible in the photo captured. (No Cap / Sun
glasses allowed).

 Always click picture in White background

 No Reflection on Face should be captured

 Only resident’s front profile is accepted in the photo uploaded.

Gender:

 Gender of the resident is matching with photo and physical appearance.

*(Change in Gender allowed only once in lifetime beyond which case needs
to be referred to UIDAI RO through 1947 toll free number for exceptional
process after update)
Age:

 Age of the resident is matching with photo and physical appearance.

*(Change in Date of Birth is allowed only once in lifetime beyond which


case needs to be referred to UIDAI RO through 1947 toll free no for
exceptional process after update)

II Document Checklist:

 PoI/ PoA/ PoR/ DoB is as per UIDAI’s Approved document list.

 Photo of the resident is matching with POI.

 Check for document validity (Issued by foreign country/


expired/cancelled are not allowed).

 Check for overwriting / correction/ Forged/ whitener used on document.

III. Checks for Annexure-I (UIDAI Standard Certificate Format):

 Check for date of issue (Valid for 3 months)

 Relevant Boxes are checked (Enrolment/ Updation, Resident / NRI)

 Aadhaar number is there in case of Update.

 Resident Signature or Thumb Impression in the relevant box.

 Check for Certifier Name, Designation, Address and Contact Number.

 Check for Certifier’s Crossed sign and Stamp on Resident Photo

 Check for Certifier Stamp and Signature in relevant box.

 Check if relevant box is selected for type of certifier (i.e. Gazetted Officer -
Group A / B & MP/ MLA/ MLC/ Municipal Councilor other…..)

 Annexure is not issued “on Behalf of” / “On proof of”.


 No overwriting / correction/ whitener used on certificate format - All
details are clear and readable.
