SUBJECT: TO CONDUCT PROPER INVESTIGATION INTO FOOD HANDLING AND
PREPARATION AT ANGELS BURGER

This is to address the incident today, February 7, 2024, that the Rural Health Unit received nine (09)
Grade 6 students from Lobo Elementary School. It was reported by Mr. Rodelio Alvarez, RN, MAN,
that the students vomited after five minutes after consuming the burger that they received as a
reward from their teacher. I would like you to be in charge of the investigation into food handling
and preparation at Angels Burger in Poblacion, Lobo, Batangas.
