I, Dogiparthi Janaki Ramulu, S/o Anjaiah, Age about: 62 Years,
Occ: Business (Kirana Shop), Native of Munipampula Village,
Ramannapet Mandal, Yadadri Buvanagiri District. Temporarily Residing
at RTC Colony, Nalgonda Town and District. Do hereby state on oath as
follows.

1. I am the Deponent herein and Claimant in main case, as such, I am

well acquainted with the facts of the case.

2. I submit that, I am originally native of Munipampula Village of

Ramannapeta Mandal, Yadadri Bhuvanagiri District. I was running
Kirana Shop Business in my village, further, in pursuance of extension of
my business I had shifted to Nalgonda in the year 2022. Further one
fateful day of 02-October,2022 I went to Chityala Town in the course of
my business collection on my motor bike bearing number TS-30-F-8453
at about 15:00 hrs while I was crossing Chityala cross road, the taker
bearing number: GJ-12-BJ-5004 belonging Respondent No.1 driven by
its driver in a rash and negligent manner without following the road and
transport rules, dashed my motor bike. As a result I fell down on the road
and my bike was totally damaged further I received multiple fractures and
grievous injuries on my right leg and body. Immediately I was joined in

private hospital at Chityal by one Medishetty Sravan. As I was serious

condition with heavy bleeding, I was shifted to Nalgonda and I was
admitted in Leelavathi Hospital under the care of orthopedic Doctor and
surgeon for better treatment. The said doctor at the above said hospital
had operated and treated me as an inpatient and I was discharged on 05-
10-2022 but the doctor was advised for continues treatment and
periodical review and bed rest for 90 days. Further my brother named
Dogiparthi Subhash gave a complaint at Chityala Police station on 08-10-
2022.

3. It is submitted that, I incurred an expenditure of Rs.5,60,000=00

towards my operation, treatment, medicines and extra nourishment.

4. The said accident occurred exclusively due to rash and negligent

driving of the driver of the crime vehicle. That the Police Chityal have
registered a case in crime no. 195 of 2022 dated 08-10-2022 under
section 337 of I.P.C. further filed a charge sheet vide CCNo. 2331 of
2022 U/s 338 I.P.C against the Driver of the tanker bearing No: GJ-12-
BX-5004.

5. Prior to the gruesome accident, I was hale and healthy, I was

carrying on my business perfectly, and I was earning an amount of Rs.
40,000=00 per month. Due to the above gruesome accident, I was
hospitalized and had to bear pain and suffering apart from the monitory
expenditures. I was contributing my entire earning to my family, except
the said earning there is no other source of income to my family. On
account of sudden accident my family and I suffered a lot and still we are
in shock as I lost my earnings. The said caused injuries of me are not
compensated in any other manner and it caused for lot of mental agony,
physical suffering and financial difficulties to me.

6. I submit that I spent Rs. 1,85,000=00 towards travelling charges

from one hospital to other hospitals, medical treatment, medicines and

extra nourishments and others. Further I lost my cell phone and bike
Therefore I am claiming compensation of Rs. 5,60,000/- including
compensation for pain and suffering, loss of expectation of my life and
amenities.

I, Dogiparthi Janaki Ramulu S/o Anjaiah, Aged: 60 Years, Occ:-

Business, N/o Munipampula Village, Ramannapet Mandal, Yadadri
Bhuvanagiri District. Temporarily Residing At: 8-1-798, RTC Colony,
Nalgonda Town & District, do hereby solemnly affirm and state on oath
as follows.

1. I am deponent herein and claimant in the above O.P and as such, I

am well acquainted with the facts of the case.

2. I have filed the above claim petition claiming compensation for the
injuries sustained by me in the Motor accident. While I was crossing at
cross road of Chityala on my Bike bearing No: TS-30-F-8453, the tanker
number GJ-12-BX-5004 of the Respondent driven by its driver in a rash
and negligent manner dashed against my Bike. As a result my Bike was
totally damaged and I sustained injuries all over my body and multiple
fractures on my right leg. Immediately I was admitted local private clinic
and for better treatment I was shifted Nalgonda and admitted in
Leelavathi Hospital, Nalgonda and I was operated by orthopedic surgeon
Dr. K. Vijay Kumar.

3. We further humbly submit that we could not file below mentioned

important documents, There was no willful latches on our part in not filing
such documents at the earliest point of time.
