    I/We, the parents/guardians named below undertake and agree to hold blameless from and against any
    loss, damage or injury caused by the participant as a result of being a part of the activities. Thus, the CAMP
    STAFF & CREW are held liable to any damages or injuries during the event and to provide the necessary
    medical attention as the need arises. Furthermore, I shall not hold the CAMP STAFF & CREW responsible to
    any untoward incident that may happen if the participant leaves the camp premises without any permission to
    his/her pastor or any of the CAMP STAFF & CREW.

    loss, damage or injury caused by the participant as a result of being a part of the activities. Thus, the CAMP
