1. Amlodipine 5mg OD HS Amlodipine is a medication commonly


prescribed to treat high blood pressure
(hypertension) by relaxing blood vessels,
making it easier for the heart to pump blood.
Additionally, it is also used to alleviate chest
pain (angina) by improving blood flow to the
heart muscle.

2. ASA (Acetylsalicylic Acid) 80mg OD ASA, or Acetylsalicylic Acid, is commonly


used as a pain reliever and fever reducer. It is
also known for its anti-inflammatory
properties and is widely used to alleviate
symptoms of various conditions, including
headaches and arthritis.

3. Ciprofloxacin BID in a week Ciprofloxacin is an antibiotic that is


commonly prescribed to treat bacterial
infections. It works by inhibiting the growth
of bacteria, helping to alleviate symptoms and
promote recovery.

4. Losartan 50mg OD at am Losartan is commonly used to treat high blood


pressure (hypertension) by relaxing blood
vessels. It is also prescribed to reduce the risk
of stroke in patients with hypertension and to
manage certain types of kidney disease.

5. Isosorbide mononitrate OD To treat certain heart conditions, particularly


angina pectoris. It belongs to a class of drugs
known as nitrates. Isosorbide mononitrate is a
vasodilator, which means it works by relaxing
and widening the blood vessels, leading to
improved blood flow and decreased workload
on the heart.

6. Atorvastatin 20g BID Atorvastatin is a medication commonly


prescribed to lower elevated levels of
cholesterol in the blood. It belongs to a class
of drugs known as statins and works by
inhibiting the production of cholesterol in the
liver, helping to reduce the risk of
cardiovascular diseases.

7. Fenofibrate 200 mg OD at am Fenofibrate is commonly prescribed to lower


elevated levels of triglycerides and cholesterol
in the blood. It works by increasing the
breakdown and elimination of fats, helping
reduce the risk of cardiovascular diseases.

8. Metformin + Sitagliptin OD Metformin + Sitagliptin is a combination


medication commonly prescribed to manage
type 2 diabetes. It works by improving insulin
sensitivity and regulating blood sugar levels in
individuals with diabetes.

Exercise

● Encourage the patient to do exercises like walking and stretching during the recovery
phase.
● Advised the patient's to avoid strenuous activity during the recovery period.

Treatment

Advised the patient’s to have enough time to rest. Provided clear instructions for the timely
consumption of medications. Encouraged loved ones to offer emotional assistance to the patient.

Hygiene

● Encouraged SO and the patient to always maintain a clean and safe environment.
● Educated the patient with the proper health care and personal hygiene.

Outpatient

● Instructed the patient and SO to return for a follow up checkup within the Doctor’s Order.
● Consulted the physician for any complications encountered.

Diet
● Instructed the patient to continue eating habits of vegetables and fruits, avoid fatty and
salty foods.
● Encouraged the patient to maintain the oral fluid intake of at least 2 liters per day.

Spiritual

● Explained to the patient and SO the importance of praying and having faith.
● Encouraged the patient and the SO to continue the strong communication with God.
● Encouraged patients and SO to attend physical or virtual mass to strengthen their faith
especially in trying times.
