I would like to ask permission to please allow me to conduct a survey among

students of Dr. Gerardo Sabal Memorial National High School, Claveria
Misamis Oriental. This is in view of my research, entitled, “Effectiveness
of Blended Approach amid COVID 19 Pandemic”

This survey would last only about 10 – 15 minutes and would be arranged
at a time convenient to the students’ schedule like during break time.
Participation in the survey is entirely voluntary and there are no known or
anticipated risks in this study. All information provided will be kept in
utmost confidentiality and would be used only for academic purpose only.
