Instructions

1. This appraisal summarizes performance throughout the period under consideration. Carefully appraise the
employee’s performance in all categories. Please be objective and provide specific details or comments which
support your ratings.
2. Discuss the appraisal with your supervisor for his/her input and agreement prior to your discussion with the employee.
3. Meet with the employee and discuss the appraisal in detail. Both of you should sign and date the form. The
employee’s signature does not indicate his or her agreement with the appraisal, but merely that he or she has read it.
Give a copy to the employee and keep one for your records.
4. Send the appraisal to your supervisor, who will also sign it and forward it to the Administrative and (for Regional
Office) or the Administrative Manager of the country.
