(RLE) POLICIES (Revised 2022)
I. UNIFORM
Students are required to be in their prescribed clinical uniform whenever in the community or in the clinical areas
A. Female student
1) In the hospital
i. Uniform prescribed by the College, tailored white dress with white apron for BSN,
tailored peach dress with apron for the DM.
ii. Plain, white/ecru chemise
iii. White stockings
iv. Nursing cap with CNHS logo (for the BSN students)
v. Nameplate
vi. Wrist watch with second hand in black color, with simple and plain faceplate
vii. White clinical shoes prescribed by the college.
2) Scrub suits
i. Scrub suit with PSU logo on the breast pocket prescribed by the College, tailored peach
top and parts for the BSN, tailored old rose top and pants for the DM
ii. Plain, white/ecru chemise
iii. Nameplate
iv. Wristwatch with second hand in black color, with simple and plain faceplate
v. White clinical shoes prescribed by the College for outside the special areas
vi. Rubber slippers for inside the special areas
3) In the community Setting
i. Prescribed Community Health Uniform, tailored white and blue pinstripe blouse with
collar and navy-blue pants for BSN, tailored peach blouse with collar and navy-blue
pants for GM. Uniforms are subject to change as per approval of college base
academic council.
ii. Nameplate
iii. Wrist watch with second hand in black color, with simple and plain faceplate
iv. Black leather shoes prescribed by the College
v. Black long umbrella
vi. Drinking water in a clean tumbler.
vii. OB/PHN bag with the following required contents:
Paper lining

B. Male students

vi. White clinical rubber-soled duty shoes prescribed by the College
2) Scrub suits
i. Scrub suit with the PSU logo on the breast pocket prescribed by the College, tailored
peach top and pants for the BSN, tailored old rose top and pants for the DM

iii. Nameplate
iv. Wristwatch with second hand in black color, with simple and plain faceplate

vii. 1 set of cap and mask, 1 white smock gown (prescribed design)

3) In the Community Setting


i. Prescribed Community Health Uniform, tailored white and blue pinstripe blouse with
collar and navy-blue pants for BSN, tailored peach blouse with collar and navy-blue
pants for GM. Uniforms are subject to change as per approval of college base
academic council.
ii. Nameplate
iii. Wrist watch with second hand in black color, with simple and plain faceplate
iv. Black leather shoes prescribed by the College
v. Black long umbrella
vi. Drinking water in a clean liquid container
vii. OB/PHN bag with required contents (see above-mentioned)

II. PERSONAL HYGIENE AND GROOMING


a. All students are expected to maintain good personal hygiene and grooming.
b. On wearing of uniforms:
i. Female students should wear uniforms have length that are 2 inches below the knee, not too
tight-fitting that would tend to restrict movements nor expose unnecessarily any part of the body
while giving care to patients or clients
ii. All students are expected to wear clean uniforms at all times
iii. Dining in public, shopping or going to market and performing social functions while in RLE
uniforms is strictly prohibited
iv. The nurse’s cap is only worn inside the ward while on duty. If the student nurse leaves her
station, she should remove her cap and place it in their appropriate bag. The nurse’s cap should
be well-starched and ironed.
v. Special areas uniform (scrub suit) is only worn while on duty inside special areas (Operating
room, Delivery room, Intensive Care Unit, Neonatal Intensive Care Unit). The student can either
change into the hospital uniform or use a smock gown and clinical shoes whenever she/her
leaves the special areas.
vi. Shoes must always be clean and well-polished, without dirt smudges.
c. Hair style and grooming
vii. Female students:
1. Adopt a hairstyle that is both becoming and convenient to manage; hair dye is strongly
prohibited
2. Long hair must be put up in a neat, netted bun at the bony at the back of the head
3. Should the female students decide to have bangs, these should not reach the
eyebrows, and must be pinned along with longer hair

4. Use of colored hair accessories are not allowed. Plain black hair accessories must be
used
viii. Male students:
1. Clean shaven facial hair
2. Males must adopt a Barber’s Cut or Clean Cut
3. Hair must not touch the ears and the nape, hair dye is strongly prohibited
b. Jewelries such as earrings, necklaces or bracelets should not be worn while on duty, except weddings
rings or school pins
c. Female students should use or apply natural-looking moderate make-up
d. Male students should never use any form of make-up except face powder
e. Fingernails must be kept short and clean. Nail polish of any color is not allowed
f. All students are required to wear their nameplate and school pin while on duty. These must be attached to
the left breast-pocket and displayed conspicuously at all times for proper identification. Nameplates and
ID cards should never be altered and defaced. Only the owner is allowed to wear her own nameplate or
ID card
g. Use of anti-perspirant is strongly advised.
h. Fragrance of any type should never be worn while on duty
i. Daily bathing, regular brushing of teeth and other hygienic practices are expected.

III. USE OF MOBILE PHONES AND ELECTRONIC GADGETS


1) Student nurses are not allowed to use the mobile phone in the clinical area except when there is permission from
the clinical instructor.
2) Cellular/Mobile phones should be adjusted to the silent mode at all times.
3) Pocket radios, and similar electronic devices are not allowed while on duty.

IV. OTHER AREAS OF CONCERN


1) Students are prohibited from receiving/entertaining visitors while on duty.
2) Students while on duty are prohibited from entering vacant/unoccupied rooms unless reason for entering is official
or justifiable.
3) Loafing from one unit to another is not allowed
4) Students are not allowed to go to the wards after their duty hours. When visiting the hospital, permission shall be
taken from the Dean of Nursing or the Clinical Coordinator. The student is required to accomplish a hospital visit
form. This is to be shown to the staff nurse on duty.
5) Students should always inform the Clinical Instructor/Staff Nurse when they leave the ward for their classes,
snacks or meals.
a. Snack break - 15 minutes.
b. Meal break - 30 minutes
6) All relationships in the ward must be maintained at a professional level. Students are allowed only one (1) snack
and one (1) meal break for an 8-hour duty schedule.
7) Students should minimize unnecessary noise in the ward because this may disrupt the resting periods of patients.
8) Students are not allowed to SMOKE during snack and meal break.
9) Students should minimize unnecessary noise in the clinical/community areas
10) If clinical instructors will not be around within 15 minutes from the start of duty, students should inform the clinical
coordinator/level head/dean’s office.

V. RLE Guidelines for the administration of Student Discipline


For students’ misconduct in RLE areas, guidelines for the administration of student discipline will be based on
provisions in the University Student’s Handbook.

PROCEDURE FOR REPORTING COMPLAINTS

1. Student to Clinical Instructor. The student is required to submit a complaint letter addressed to the College Dean through
RLE Coordinator/Department Chairperson.

2. Student to Student. The student is required to submit a complaint letter addressed to the Adviser.

3. Clinical Instructor to Student. The CI is required to submit a complaint letter addressed to the College Dean through RLE
Coordinator/Department Chairperson.

VI. CLINICAL DUTY ATTENDANCE – GENERAL DIRECTIVES


A. TARDINESS
i. A student should report 15 minutes before the start of the official time (call time). A student considered
tardy if he/she arrives less than 15 minutes before the start of the official time

ii. Types of Tardiness


1. Regardless of the type of tardiness, the student will be given a student slip
2. Excused tardiness – tardiness due to a “force majeure” such as serious illness, typhoons and
other natural phenomena and emergencies involving the family.
3. Unexcused tardiness- the tardiness is due to dysmenorrhea, simple, colds, running errand for
somebody, studying for examinations, visiting friends, waking up late, loitering in the hospital or
for other reasons not listed in the circumstances which make the tardiness excused.

iii. Guidelines Observed In Case of Tardiness


1. A student who incurs excused tardiness, is required to present to the Clinical Instructor an
excuse letter by his/her parents the following day of his/her duty, this excuse letter must be
signed and approved by the Dean. He/she shall be admitted for duty but the Clinical Instructor
records tardiness. Failure to present said letter will mean unexcused tardiness.

iv. Cumulative Tardiness


1. A student who incurs 3 unexcused tardiness in 1 semester is considered one day equivalent of
duty (3 days unexcused late: 1 day make up duty)
Note: students who reported tardiness will be given student slip by the Clinical instructor
which will be forwarded to the RLE Coordinator and to the College Dean. If the reason/s
for is disputable, explanation letter must be given by the student, noted by the
parent/guardian and forwarded to the RLE Coordinator and to the College Dean.
B. ABSCENCES
i. A student is considered absent if:
1. He/she fails to report for clinical duty in his/her assigned area without any prior notice.
2. He/she came to duty 1 minute after the official duty time

ii. Types of absences


1. Excused absences are those incurred because of severe illness of the student and/or on family
members like parents, sisters and brothers, the student should present medical certificate along
with excuse letter. In case of death of immediate family/household members, the student is
excused from his/her duty for a maximum of 4 days only. “Force majeure” such as serious
illness, typhoons and other natural phenomena and emergencies involving the family are also
considered excused. The student must present excuse letter committing absence.
2. Unexcused absences are those which incurred due to invalid reason and for other reason not
listed in the circumstances which make the absent excused. Reasons for unexcused absences
includes performing activities not included in the Plan Of Activities of the clinical exposure,
review for scheduled exam, fiesta/cultural reason, attending weddings and other social functions,
family reunions, dysmenorrhea, illness without medical certificate, extension of vacation, and etc.
3. Guidelines concerning absences during clinical duty
i. If the student is absent, excused or unexcused, the student should present an
explanation letter signed by the student and his parent/s and/or guardian. This letter
must be first submitted within 3 working days to the RLE Coordinator for endorsement,
and forwarded to the Dean’s Office for approval. If the absence is due to illness, Medical
Certificate with formal and official heading and not the prescription paper of the Attending
Physician must be attached. The medical certificate will undergo the scrutiny of the RLE
Coordinator, College Secretary and the Dean.

ii. If the student is absent during orientation, he/she is required to make up for 3 days
equivalent of duty hours, however for some special affiliated hospitals, NO
ORIENTATION NO DUTY policy will be applied, the student is not allowed to duty on
that hospital.
iii. If a student is absent during RLE major exam, the student should present a letter of
explanation to the clinical instructor and to be noted by the clinical coordinator, only then
the student will be allowed to take the missed exam/s.
iv. Make up duty ratio of excuse absence is 1 day excused absence to 1 day equivalent of
duty hours.
v. Make up duty ratio of unexcused absences is one day unexcused absence to 3 days
equivalent of duty hours (1:3)
vi. Whether the absence incurred is excused, the student should present an excused letter
attested by the parent to the Clinical Instructor, otherwise he cannot be admitted back to
duty.
4. Repeated and habitual Tardiness/absences
i. For unexcused absences, a letter of explanation to the Dean is required. This serves
as a first warning to the student. If the above act is repeated, a student writes a letter
of explanation to the Dean and his/her parents should be summoned and warned that
the third offense would mean failure in the RLE/Clinical Duty. If the student incurs
actual unexcused absences for a total of 20% of the total RLE/Clinical Duty hours in
semester, the students automatically fail in the RLE/ Clinical Duty hours in semester,
the students automatically fails in the RLE/ Clinical grades.

VII. MAKE-UP DUTY GUIDELINES GENERAL DIRECTIVES


Once the student has given a make-up duty due to guidelines stipulated in this manual the following guideline will
be used.
1. All make-up duty/ies scheduled by the RLE Coordinator are strictly followed
2. A student must pay the required amount for make-up duty. NO RECEIPT NO MAKE UP DUTY.
3. All make-up duty is scheduled during weekends, semestral, Christmas, and summer breaks
4. A student is not allowed to graduate if he is not cleared in his entire make-up duties
5. A student allowed having a make-up duty for a maximum of 8 hours per day only.
6. If the student is absent during the scheduled make-up duty, the student will be given another
schedule for make-up duty
a. Excused absent: 1:1
b. Unexcused absent: 1:3
c.
VIII. Sanctions of violation on policies aside from tardiness and absences:
A. First offense – verbal warning with submission of incidence report
B. Second offense – written warning from the Dean’s Office, duly noted by the student’s parents/ guardian
C. Third offense – the student will be endorsed to the Office of the Student Affairs for proper actions.

IX. GUIDELINES FOR ON- CALL DUTY


i. A student must have completed the number of cases required by the College and mandated by the
Professional Regulation Commission prior to graduation.
ii. A student who has not complied with the required number of cases, any time prior to graduation, must request
in writing to the College Dean for an On-call duty subject for approval.
iii. Any approved On-call duty in a particular clinic or hospital should have a direct supervision of a clinical
instructor.
iv. Cases taken from accredited institution will not be credited as an official case if done without the presence of a
clinical instructor.
v. On-call duties are scheduled by the Clinical Coordinator subject to the availability of the Clinical Instructor.
vi. On-call duties must not be done during regular duty hours.
vii. A student/s who requested for an On-call duty must pay the assigned Clinical Instructor as an overtime
payment prior to the scheduled date.
viii. Any form of payment will not be accepted in the College. All details of On-call duty including the schedule and
amount of payment must be submitted by the RLE Coordinator to the University Cashier.
ix. The maximum number of students for the On-call duty must only be up to three (3)/ per shift to insure handled
cases.

X. ERRORS AND MISTAKES
A. Errors and Mistakes
i. Errors and mistakes in the RLE area should be reported immediately to the CI
ii. All cases shall be investigated following the proper channel of communication
iii. The student shall then write an incident report submitted to CI concerned within 24 hours, IR will be forwarded to
the Dean through the RLE Coordinator/Chairperson for comments and recommendation.
B. Major Offenses
Any of the following constitute a major offense:
i. Tampering of RLE record and/or patients’ records
ii. Signature forgery (CI, Doctors, Deans & Asst. dean)
iii. Stealing in any form including misappropriation of funds entrusted by a group or by other students
iv. Administration of drugs that produce harmful effect on the patient, resulting in complications or death due to error.
v. Viewing of pornographic films or materials inside the hospital premises
vi. Grave disrespect for and causing physical harm to clinical instructors.
vii. Execution of nursing procedures without the knowledge/supervision on the clinical instructor.
viii. Sleeping while on duty.

C. Sanctions
1st major offense – warning + 5 days extension of duty with incident report (Extension of duty will be scheduled after the
regular duty, students must pay assigned CI prior to scheduled duty extension through the university cashier)
2nd major offense – 10 days extension with incident report
3rd major offense – referral to the Disciplinary Officer for sanction.

XI. GUIDELINE FOR CLINICAL GRADUATION


For the student to be a candidate for Clinical Graduation, he/she must:
1. Satisfactorily submitted all the requirement on all areas of all RLE subjects.
2. Completed all required number of hours of RLE subjects.
3. Obtained a grade of not less than 2.25 or 80% in all RLE subjects.
4. Completed the required number of Surgical and / or Delivery PRC cases and submitted duly signed forms.
