    Myself Sarfaraz Patel most humbly beg & inform your Honor that as per
    my previous frequent request regarding the illness of my old aged parents
    I was transferred & placed at Old Civil Hospital,Surat vide
    Order no.98/K-3/2022 did.16/06/2022.

    It is regretted that within a short span of time myself received an order


    of Deputation for SDH Mandvi, Surat.

    As my parents are old aged, my mother is a diabetic patient, suffering


    from hypertension and unable to walk independently, it is most humbly
    requested to instruct the concern officer for the suitable arrangement
    of Jr.pharmasist for SDH MANDVI &issuance of an Order for me to perform
    duty at OLD CIVIL HOSPITAL, SURAT.So that myself can be able to spare
    more time to take care of my old parents and my family as well.
