Subject: Mandatory Use of Malaffi Web Forms for
Applicable JAWDA Key Performance Indicators by
Designated Healthcare Facilities Staff
We would like to extend you our greetings, wishing you
all the best and success.
In our continued efforts to enhance patient care and
streamline healthcare processes, the Department of
Health - Abu Dhabi is automating the process of
gathering specific Jawda clinical key performance
indicators utilizing Malaffi health information exchange
platform.
Thus, we stress on the mandatory utilization of Malaffi
web forms to capture missing data elements related to
specific Jawda performance KPI as part of initial phase of
the Jawda automation process, aiming at enabling
automation of data collection and reducing the
operational burdens for facilities.
All healthcare facilities must complete and submit the
applicable webforms with missing data within one month
from the form creation date.
Noting that healthcare facilities shall bear the full
responsibility for accurately entering the above-
mentioned data elements within the requested timeline,
as this will directly impact the outcomes for JAWDA Key
Performance Indicators (KPIs).

‫ويجب على جميع منشآت الرعاية اكمال النماذج املعمول بها وتقديمها‬
