Request for borrow of equipment

I, JERALD L. COLETE, Member of the Sangguniang Barangay of Barangay San Juan, Sta
Rita Samar, together with REXTER L. LACABA, as Youth Representative, are requesting to borrow
the DRUM and Lyre equipment’s listed below, from the MAPEH Department of your School for the use
of upcoming Christmas Activities of the Youth and Sangguniang Barangay of Barangay San Juan,
Sta. Rita Samar. I will take care of it and return it in Decemeber 26, 2023, in good condition. We also
assure you to repair or replace the damage equipment’s if ever.
