PROMISSORY NOTE

MR. ARNOLD ACALA MELCA of BRGY. SANTA ISABEL, McARTHUR,

LEYTE, hereinafter known as the PAYOR promises to pay to MASTER DELTA
FORCE SECURITY AGENCY, represented by its proprietor, MR. PEDRO C.
DELFIN, hereinafter known as the PAYEE, the principal sum of FIFTEEN
THOUSAND PESOS (Php 15,000.00), in the manner that follows:

1. PAYOR shall pay in installment basis the abovementioned principal

amount as payment for the lost 38 caliber gun under his watch around
3:00 AM, while posted at Toyota, Brgy. 71, Naga-Naga last June 29, 2020.

2. The installment refers to a deduction of One Thousand Pesos (Php

1,000.00) per month or Five Hundred Pesos (Php 500.00) twice a month
from the PAYOR’s salary starting on the month of July 2020.

3. The outstanding principal balance shall become immediately due in full

upon transfer of employment of the PAYOR.

4. Nonpayment of the outstanding principal balance in full will constrain the

PAYEE to take legal action in the enforcement of the Promissory Note.

5. The deliberate intention to evade the obligation will further constrain the
PAYEE to report the PAYOR to the Regional Civil Security Unit (RCSU) in
Campetic, Palo, Leyte.

IN WITNESS WHEREOF, I set my hand under seal this ____ day of July, 2020,
and I acknowledge receipt of a completed copy of this instrument.
