This is to formally present our proposal to undertake the supply of PS/PC piles in your project as stated below:

I. SCOPE OF WORK

ll. PAYMENTS TERMS

1. 30% down payment before fabrication PS/PC concrete pile.
2. Full payment before delivery.
III. TERMS AND CONDITION
The client shall:
3. Clear right of way for free access of CSI equipment. Access and working area shall be prepared in all working condition which can support the unloading of
piles.
4. Evaluate and approved the submitted drawings and specification as basis of submitted price quotation. Change of specifications aftermath of evaluation and
checking the price will also change correspondingly.
5. Assign an authorized representative to witness concrete pouring of the ordered piles, in the conduct of testing and breaking of samples.
6. Bonds and insurances are not included in the contract. All types of permit needed will be the responsibility of the client.
7. Crane for loading the piles from to Client’s truck will be provided by CSI.
CSI shall:
8. Fabricate the piles in accordance with the plan, drawings and specifications evaluated and approved by the client.
9. Fabrication shall commence 7 days from receiving of the Down Payment.
10. Compressive testing of cylinder samples shall be done on CSIs own testing laboratory.
Other Conditions
11. The driven piles will eventually be used as a part of the foundation of the building or structure for which it is intended being the OPTION specified by the
clients ‘DESIGN ENGINEER’. CSI is being supplier of piles shall no responsibility if the building or structure will sustain any defects, damage or structural failure.
12. Pile Dynamic Analysis (PDA) and Pile Integrity Test (PIT) are test that are usually undertaken on driven piles. It shall be understood that the responsibility of CSI
on the piles is limited only on the fabrication. Hence, If piles failed during conduct of PDA and PIT test, the cause/s are beyond the control of CSI and therefore
not its responsibility, any additional piles required by the client that incur additional cost should be consider as additional work of CSI.
13. Price validity: 30days from the date above.
