Using the CPAT to measure interdisciplinary collaboration before and after implementation.

Instructions:
Be sure that your plan addresses the following, which corresponds to the grading criteria in the scoring
guide. Please study the scoring guide carefully so you understand what is needed for a distinguished
score.

 Explain an organizational or patient issue for which a collaborative interdisciplinary team

approach would help achieve a specific improvement goal.

 Summarize an evidence-based interdisciplinary plan to address an organizational or patient

issue.

 Explain how the interdisciplinary plan could be implemented and how the human and financial
resources would be managed.

 Propose evidence-based criteria that could be used to evaluate the degree to which the project
was successful in achieving the improvement goal.

 Communicate the PowerPoint presentation of the interdisciplinary improvement plan to

stakeholders in a professional manner, with writing that is clear, logically organized, and
respectful with correct grammar and spelling using current APA style.

This is the suggested outline:

 Part 1: Organizational or Patient Issue.

 What is the issue that you are trying to solve or improve?

 Why should the audience care about solving it?

 Part 2: Relevance of an Interdisciplinary Team Approach.

 Why is using an interdisciplinary team relevant, or the best approach, to addressing the
issue?

 How will it help to achieve improved outcomes or reach a goal?

 Part 3: Interdisciplinary Plan Summary.

 What is the objective?

 How likely is it to work?

 What will the interdisciplinary team do?

 Part 4: Implementation and Resource Management.

 How could the plan be implemented to ensure effective use of resources?

 How could the plan be managed to ensure that resources were not wasted?

 How does the plan justify the resource expenditure?

 Part 5: Evaluation.

 What would a successful outcome of the project look like?

 What are the criteria that could be used to measure that success?

 How could this be used to show the degree of success?

For this assessment you will create an 8-12 slide PowerPoint presentation for one or more stakeholders
or leadership groups to generate interest and buy-in for the plan proposal you developed for the third
assessment.

As a current or future nurse leader, you may be called upon to present to stakeholders and leadership
about projects that you have been involved in or wish to implement. The ability to communicate a plan
—and potential implications of not pursuing such a plan—to stakeholders effectively can be critically
important in creating awareness and buy-in, as well as building your personal and professional brand in
your organization. It is equally important that you know how to create compelling presentations for
others' delivery and ensure that they convey the same content you would deliver if you were the
presenter.

Slide 2: Introduction - Organizational or Patient Issue

 Speaker's Notes: Ineffective communication in healthcare organizations leads to medical errors,

increased patient recovery time, and decreased staff satisfaction (Schot et al., 2020). Addressing
communication issues is vital to enhance patient care, safety, and organizational efficiency. Our
goal is to improve healthcare quality by promoting interdisciplinary collaboration, which has
proven effective in enhancing organizational communication (Herawati et al., 2022).

Slide 3: Relevance of Interdisciplinary Approach - Part 1

 Speaker's Notes: An interdisciplinary team comprises professionals from various disciplines

working collaboratively. This approach is essential for addressing communication issues as it
integrates diverse expertise and perspectives, leading to comprehensive and effective solutions
(American Journal of Nursing, n.d.). Schot et al. (2020) emphasized that healthcare professionals
significantly contribute to interprofessional collaboration, which is critical in communication
improvement.

Slide 4: Relevance of Interdisciplinary Approach - Part 2

 Speaker's Notes: Using an interdisciplinary approach can lead to improved care coordination,
fewer medical errors, and enhanced patient safety (Herawati et al., 2022). Additionally, this
approach optimizes resource utilization and increases employee engagement and job
satisfaction, significantly improving healthcare outcomes and efficiency (Sigmon, 2020).
Fostering a culture of collaboration is essential for success.

Slide 5: Interdisciplinary Plan Summary - Objective

 Speaker's Notes: The primary objective is to establish regular team meetings, implement shared
communication platforms, and develop standardized communication protocols (Sigmon, 2020).
Utilizing technology and combined strategies reduces ambiguity regarding duties of team
members.

Slide 6: Interdisciplinary Plan Summary - Predictions

 Speaker's Notes: With improved communication and collaboration, better care coordination,
fewer medical errors, and enhanced patient safety is expected (Herawati et al., 2022).
Addressing potential barriers with change management strategies, leadership support, and staff
education is essential for successful plan implementation (Sigmon, 2020). Participation, learning,
and adaptability will instill a sense of ownership and commitment among team members.

Slide 7: Implementation & Resource Management - Part 1

 Speaker's Notes: Implementation of this plan will be systematic, utilizing the PDSA model and
transformational leadership strategies (Kay et al., 2023). Existing communication equipment and
technologies will be leveraged for cost-effectiveness and resource optimization (MacPhee et al.,
2020). Staffing needs have been considered, and additional interdisciplinary team members are
not required.

Slide 8: Implementation & Resource Management - Part 2

 Speaker's Notes: Use of the synergy tool will determine staffing needs and ensure that resources
are not wasted (MacPhee et al., 2020). Implementation can prevent potential costs related to
poor communication, such as penalties and litigation (Burke et al., 2018). Resource expenditure
is justified by aiming to improve patient outcomes and staff satisfaction (Olley et al., 2019).

Slide 9: Evaluation

 Speaker's Notes: Successful outcomes include improved interdisciplinary communication,

improved patient satisfaction, and efficient resource use. The Collaborative Practice Assessment
Tool (CPAT) questionnaire will be used to measure improved interdisciplinary communication
(Herawati et al., 2022). HCAHPS scores and other tools will be used to measure patient
satisfaction (Rodriguez-Homs et al., 2020). These metrics, along with reduced medical errors and
improved communication, can demonstrate the degree of success to stakeholders (Ogundeju,
2020).

Slide 10: Conclusion

 Speaker's Notes: This interdisciplinary plan provides a comprehensive approach to improving

communication within healthcare organizations. Commitment, collaboration, and continuous
improvement efforts are required to foster a culture of excellence in healthcare delivery. By
promoting effective communication and collaboration, an improved work environment for
healthcare professionals and patient is achieved.
