MONTHLY ACCOMPLISHMENT REPORT IN GRADE III


As of September 2022

I. Introduction

As a teacher of Alalay Grande Elementary School, I have accomplished the following activities to
improve instructional competence for the month of September 2022.

II. Accomplishments

A. Learners’ Development

All Grade III learners were enrolled in the Face-to-Face Modality. The following are the activities
undergone and accomplished:

a. Enrolled 15 males and 12 females in the LIS.


b. Took height and weight for nutritional status assessment.
c. Collected survey form from learners on their learning resources at home.
d. Gathered individual learners’ data.
e. Conducted online meeting or orientation to parents and learners on the opening of classes
f. Participated in the orientation of SAGIP project, a division advocacy to address learning gaps among
learners.
g. Organized Homeroom PTA and Classroom Officers.
h. Joined in the activities of earthquake drill.
i. Conducted Psychosocial Support Activities to Grade III learners and parents.
j. Administered EGRA and PHILIRI
k. Administered Regional Diagnostic Test
l. Participated in the counting of votes for elected SPG Officers.

B. Teachers’ Development

a. Downloaded educational videos related to topics for Grade III learners.


b. Made inventory of available modules, LAS ,books and resources from DepEd
c. Updated SF forms .
d. Mastered enrolling learners in the LIS.
e. Prepared teacher made test/quizzes.
f. Attended meeting on the preparation of teacher coordinators’ report.
g. Prepared assigned SMEPA documents prior to district evaluation.
h. Attended district virtual orientation meeting on preparation of documents for SMEPA validation on
September 2022.
i. Accomplished and submitted documents needed in file 201.

C. Curriculum Development

a. Prepared Daily Lesson Logs (DLL) for the month of September 2022.
b. Prepared contextualized lesson plans and supplemental materials in Grade III that support the
curriculum.
c. Adapted the locally available or indigenous materials in teaching the lesson.
d. Adapted and used locally available indigenous materials and the quality assured lessons and videos from
DepEd portal in delivery of Grade III lessons.
e. Employed differentiated instructions to variety of learners.
f. Conducted pre assessment on reading vowels and consonants.
g. Organized reading group on the implementation of project LIRA (Learners’ Improvement on Reading
Ability) of the school and project SAGIP of the division.
h. Administered and submitted the Regional Diagnostic Assessment most and least learned competencies.
i. Compiled reading materials based on learners’ ability.
j. Evaluated pupils reading performance using project LIRA following the reading log quarterly.
k. Provided reading exercises on enhancement of comprehension skills using project LIRA or the project
SAGIP to my class.

D. Parents Community and involvement

a. Conducted parents’ meeting about School Based Feeding Program


b. Participated in the strategic Planning and GPTA Meeting
c. Sought support of parents on the schools’ cleaning, gardening and area beautification.
d. Involved parents in the Project LIRA of the school.

E. Others
1. Stakeholders provided cleaning materials.
