We are very sorry to inform you that Mr. Abdul Malek, Junior HR & Compliance
Executive of our company had died at 0900 hours on 08 October, 2010 the Friday at
Avicenna Hospital-Sirajganj. He was serving here for last 4 years. He was enlisted in
Group Insurance. We informed to BGMEA about this matter (Ref: Amt-HR. 08/10/12 &
Dated on October 10, 2010) and collected claim form from BGMEA. Hence we are
submitting with all documents as per claim form’s requirements for your farther necessary
action.

Therefore, you are requested to take proper steps about this matter as early as possible for
the betterment of the family of Late Mr. Malek.
