The Level III BSN students of Ateneo de Zamboanga University, are conducting a study entitled
“Awareness of Coronary Artery Disease Signs, Symptoms, and Risk Factors among East
Coast Jeepney Drivers in Zamboanga City” as a partial fulfillment of Nursing Care
Management (NCM 115j) Research. A test questionnaire will be used in the said research. In
view of the foregoing study, the researchers would like your expertise to validate the attached
adopted questionnaire to qualify for conduction. Knowing your experience in the field of
research and education, We would like to ask for your help in validating the said instrument
before administering it to the participants of this study.

STATEMENT OF THE PROBLEM:
The study aims to determine the awareness of the jeepney drivers of Zamboanga City regarding
the risk factors of Coronary Artery Disease.
Specifically, it seeks to answer the following questions:
1. What is the Knowledge, Attitude and Practice of Jeepney drivers on CAD?
