Parental /Guardian Consent

I/we hereby willingly and voluntarily give consent to the participation of my/our
daughter ALLIJAH MAE L. CATURA of GRADE 12-HUMSS in the DEVCON ROADSHOW
2024, on April 15-16, 2024, Legazpi City Exposition Center, Airport Road, Legazpi City.

This activity is in support of the request of the Hon. Geraldine B. Rosal, City Mayor
of Legazpi, City Government of Legazpi.

I have considered the benefits that my daughter will derive from her participation in
this activity provided that due care and precaution will be observed to ensure the comfort
and safety of my daughter that DepED employees and personnel may not be held
responsible for any untoward incident that may happen beyond their control.
