 Present History
- The condition started 2 years ago in the form of conception in spite
of regular marital life without use of contraception.

- Semen analysis was done 1 year ago & was fertile.

- The patient has irregular cycles ( oligo-hypomenorrhea ) since

menarche 1-2 days every 3 months, scanty or even spotting in
amount, also she has no spasmodic dysmenorrhea, no ovulatory
pain, bleeding or discharge.

- She has no history of abdominal operations, no history of fever

with bilateral lower abdominal pain & offensive vaginal discharge.

- She has no history of galactorrhea or hirsutism.

- She has investigations in the form of foliculometry, done for 3

cycles at private clinic & clomid was given in these cycles 2 tablets /
day from the 3rd day to the 8th day of the menstrual cyclegood
follicular maturation was noticed only in one cycle at the 15th day of
the menstrual cycle & IM injection was given & the patient was
advised for timed intercourse.

- Hormone profile was also done after the clomid induction but no
available results.

- She didn’t have premenstrual endometrial biopsy.

- Hysterosalpingogram, diagnostic laparoscopy, post coital test have

not been done yet.

- No treatment was given other than clomid.

- System review revealed no abnormality.

Example of routine antenatal care sheet:-
- She is pregnant ± 30 weeks as her 1st day of the LMP was on …/…/…
& so her E.D.D will be on …/…/…

- First, the patient noticed one missed period, pregnancy test was
done few days after the missed period (on …/…/… ) & was positive.

- She passed through normal first trimester a part from mild nausea,
frequency of micturition, constipation.

- These symptoms disappeared spontaneously after the first

trimester.

- She perceived the fetal movement for the first time at about 20
weeks.

- The patient has no warning symptoms as she has no as general

Example of bleeding sheet (menometrorrhagia):-

- The condition started 6 months ago when the patient noticed
change in her menstrual pattern, instead of being regular, 3 days
every 28 days, with an average of 2-3 diapers / day it became
almost continues vaginal bleeding.

- The bleeding is mild (1-2 diapers / day), bright red but sometimes it
is dark red.

- The condition is preceded & associated with colicky lower

abdominal pain radiating to the lower back, it is also associated
with scanty offensive vaginal discharge but no itching.

- The condition is not preceded by symptoms suggestive of

pregnancy.

 - The condition is not preceded by use of hormones & contraception.

- The condition is not associated with any swelling, fever or

metastasis.

- Ultrasound was done at private clinic & revealed uterine fibroid.

- Treatment was prescribed in the form of injections but the codition

didn’t improve.

- She was admitted in Kasr El Aini, investigations were done in the

form of ultrasound that revealed a uterine fibroid also liver &
kidney function tests but no available results & she is waiting for
operation.

- There is no history suggestive of blood disease ( bleeding from

other orifices, ecchymosis ) , heart, liver, kidney, or thyroid
disease.

Example of bleeding sheet (postmenopausal):-

- She is menopausal for 6 years, the condition started 20 days ago by
mild vaginal bleeding (1-2 diapers/day) almost continuous, its color
is dark red.
- It is painless & not associated with offensive vaginal discharge.
- The bleeding is not preceded by use of hormones.
- The bleeding is not associated with any swelling, fever or
metastasis.
- Vaginal ultrasound was done but she doesn’t know the results.
- Treatment was prescribed in the form of tablets but the condition
didn’t improve.
- She was admitted in Kasr El Aini, investigations were done in the
form of ultrasound, ECG, blood analysis with no available results.
- She is waiting for fractional curettage.
- System review revealed no abnormality.

‫‪which was at home by nurse midwife with excessive fundal‬‬
‫‪pressure ), when the patient noticed a mass protruding from the‬‬
‫‪vulva that increases on straining & decreases on lying down.‬‬
‫‪- The condition is (not) associated with backache more at the end‬‬
‫‪of the day.‬‬
‫‪- The patient experienced congestive dysmenorrhea, leukorrhea‬‬
‫‪& menorrhagia for 1 year.‬‬

- There is (not) associated frequency of micturition with dysuria,
sometimes the patient is unable to complete micturition unless
the mass is reduced by her fingers, but there is no stress
incontinence.
- There is no rectal symptoms.
- She thought medical advice at Kasr El Aini out patient clinic
where she was admitted 5 days ago & investigations were done
in the form of blood & urine analysis but she doesn’t know the
results.
- The patient had no previous surgical repair for this mass.
- She is waiting for surgical repair.
