Respected Sir,
Hope you are doing well, as we
discussed Verbally about Near miss which
Occurred in PM-6 in which a Piece of
Concrete fall from Valay beam number 23 as
counted from CTMP Side on Dated:-
29/03/2024 at time of 12 o'clock approx.
There are My Findings Listed Below Upon my
Initial Investigation.
1. Al-Masoom and Royal team was Working
on PM7 Roof top for Guard Rail installation
there is Gap Between PM-6 & PM-7 (Hardly
possible for Drill Vibration effect on PM-6 roof
top).
2. Today Both teams are only Permitted on
work on PM-7 only.
3. K-Solar team Did Chipping activity
(Manually) to verify Slop on 11th Valay Beam
As counted from CTMP Side. It is hard to
impact on 23rd Valley Beam.
4. Upon Investigation on PM-6 Roof top there
is No traces of Recent Activity/Chipping on
23rd Valley Beam.

Conclusion for this initial Investigation it is

recommended to Coordinate with Civil
Department for Further Investigation to Find
out the Root Cause by measuring Strength of
Roof Top and Check the Adhesiveness for the
Concrete. As you know in future there will be
heavy load and mechanical Drilling. So,
prevention for this type of Misses it is
compulsory to Coordinate with Civil Team.
