Materials, tools and equipment required: Tractor trolley, barrows, koda, shovel, mortar mix, drums for water storage
Now undertake the JSA and record in the table below. Use prompts on JSA Prompt Sheet and Hazard Checklist PA-HSEC-02-040-002-F03.
Part 3: Contingency Plans.
Clear access to be maintained to Phase 1 area and Ambulance to be positioned in Phase 1 area in the event of an emergency.
Part 4: Review. After the job has been completed, JSA leader should note: any hazards, which were identified in the original JSA? Any practices that need to be
captured in the OMS? List action items, changes, and transfer any changes to the Change Management system.

1 Mobilization of equipment, tools & Unauthorised tools brought to site Before mobilizing any tool make sure that the E &P

Unskilled and uninducted personnel starting the job Ensure that all personnel prior to start the have been
inducted and oriented about the hazard in the job site.
Workers starting the job unfit to work All personnel must undergo the fitness exam from DEL
company doctor & if not fit to work shall not be allowed
to work at the project site.
Workers starting the job without permit JSA must be in place prior to work. Ensure all the
persons take part in the toolbox meeting discussion.
All Persons must sign at the back of JSA.CWP shall
be obtained from OPS.
Throwing of Bricks Ensure that workers must not throw the bricks during
working to avoid any incident.
No access in the event of an emergency Ensure that proper access & egress was in place by
way of opening in fence at all times work activities are
in progress.
Hand injury when stacking bricks Ensure gloves are worn at all times.
2 Trimming of slope Hand injury Ensure that workers are wearing of hand gloves during
manual trimming

3 Excavation for Toe Beam Damage to buried service BHP Operations excavation certificate to be obtained
and all requirements define therein are followed.
4 Bricks laying and grouting Hand Injury Ensure proper hauling of materials/get help if
required/Wear hand gloves.
Mishandling of hazardous material MSDS of Cement must be attached with the permit
and requirements outlined followed.
5 Demobilization Area left untidy resulting in tripping hazards, etc. Ensure that area should be clean after the work is
being finished.
