4- Lifting of Dismantled ladder with crane to new location.
5- Installation of Monkey ladder at New Location by welding.
6- Demobilization

2. Construction of Foundation at the
new location of Monkey Ladder.

plan, role, responsibilities toolbox meeting and people are aware of job
and hazards involved in the job they are
doing. Jsa was discussed in the TBM and
every one is required to sign at the back of
JSA.
Heat Exhaustion Persons involved in manual labour or grinding
activities shall be provided with Drinking water
supply all the time.
SIMOPS-With Operations All works to be under taken under PTW
system.
Hand Injury. Ensure proper handling of materials/ wear gloves
during performing all activities.
Back Injury. Ensure proper lifting of material get help from

hazard. All material should be placed at safe and
proper locations to avoid any trip/fall hazard.

throwing.
Ensure that all workers of any craft must ware full
PPE’s
Prior to work grinder must be inspected by
E&P .Positive locking system shall be in place.
If sound is more than 85db use of double ear
muff protection is mandatory.
Ensure the grinder must be installed with the
safe guard.
Ensure the correct position of the grinder man
such that sparks should go towards safe
area.
Fire blanket shall be in place during the
activity.
Ensure proper hauling of materials/Wear hand
gloves.
Only certified operator should operate the
crane. A copy of the daily checklist should be
in the crane. All the rigging gears should be
certified. The operator must have his load
chart to confirm his allowable distance of load
