3- Dismantling of Monkey Ladder from supports by cutting with grinder
4- Lifting of Dismantled ladder with crane to new location.
5- Installation of Monkey ladder at New Location by welding.
6- Demobilization

Part 3: Contingency Plans. Standard PPE,S First aid kit must be present at the area and the Hse personnel is the first response in case of

that need to be captured in the OMS? List action items, changes, and transfer any changes to the Change Management system.

1 Mobilization of workers & Unauthorised equipment/tools brought to site Before mobilizing any equipment/tools make sure

every one is required to sign at the back of
JSA.
Heat Exhaustion Persons involved in manual labour or grinding

SIMOPS-With Operations All works to be under taken under PTW
system.

3 Dismantling of Monkey Ladder from Uninspected Grinder brought on site Prior to work grinder must be inspected by
supports by cutting with grinder E&P .Positive locking system shall be in place.
Unwanted sound If sound is more than 85db use of double ear
muff protection is mandatory.
Grinder without safe guard Ensure the grinder must be installed with the
safe guard.
Position of Grinder Man Ensure the correct position of the grinder man
such that sparks should go towards safe
area.
Fire blanket shall be in place during the
activity.
Hand Injury Ensure proper hauling of materials/Wear hand
gloves.
