7. RECORDKEEPING REQUIREMENTS

GENERAL

1) It is Contractor’s responsibility to maintain all records required by Owner, Government


Occupational Security Insurance (GOSI), or similar regulations. This includes the maintenance
of an accident log, annual summary and the posting of all prescribed posters.
2) A log of safety activities, accident investigations, employee instruction, training, toolbox
meetings shall be maintained onsite and copies shall be promptly provided to Owner upon
request.
3) Contractor shall maintain all Weekly Area Safety Assessments, Area Discrepancy Logbooks,
Safety Audit Reports, Supervisor Safety Logbook and make it available to Owner upon request.
4) The Safety Manager shall prepare narrative safety reports weekly and monthly and submit to the
Construction Manager and Owner Safety Manager. The Weekly Safety Report shall include all
related tabulated safety statistics and graphs to include all incidents/accidents during the
reporting period, hazards observed and corrective actions to deal with them, achievements,
initiatives, training, non-compliance issues, programs implemented and activities planned for
the next week.
The Monthly Safety Report shall be submitted to Owner Safety Manager not later than the 5th
day of each month similar to the weekly report above but in addition include an objective
overview of the past month’s activities and activities planned for the next month.
5) Accident experience records and statistics, incident rates, man-hours, number of employees will
be maintained and submitted to Owner weekly and monthly.
6) Medical facilities will maintain complete records of all illnesses and injuries and treatment
given and shall be made available to owner upon request.

7) All injuries on the project shall classified first aid, recordable, restricted and days away from
work injuries and injury incidence rates in accordance with OSHA Record-keeping Guidelines
and ANSI Standard Z16.4-1997. Incidence rates will be based on 200,000 man-hours worked.

MEDICAL RECORDS

1) The Safety Manager is responsible for insuring that the appropriate safety-related reports
concerning occupational injury/illnesses are properly executed and maintained.

2) The following is a list of those reports and records:

a. Employee Injury Report

b. Monthly Occupational Injury/Illness Report (OSHA 200 Log)

c. First Aid Log

d. Injured Employee’s Job Description

3) The Physician is responsible for maintaining accurate records of medical treatment and
providing the Safety Manager with a copy of the same. The following is a listing of these
records.

a. Doctor’s Medical Report to Employer

b. Medical Consultation Report

c. Employee Injury / Illness Report

d. Return to Work Notice

4) Drug testing will be in accordance with Saudi regulations and to consist of Substance
Abuse Prevention and Chemical Screening Program. The above regulation and policy will
apply to all expatriate personnel whether full time or part time being assigned to the project.

CONSTRUCTION MANAGER REPORTS

1) Doctor's Return to Work Notice is to be completed by the Physician and kept on file. The
Doctor's Release must be completed by the physician before the individual can be allowed
to return to work.

2) Report of Accident is to be completed by for all injuries sustained by his craftsmen


requiring medical attention. The completed report is to be copied by the end of the shift on
the date of occurrence and sent to the Safety Manager.

3) Employee Report of Injury This will be completed immediately after the industrial
injury/illness by the Employee or Safety Manager.

4) Doctor’s Medical Report to Employer is completed by the treating physician. A copy will be
made and forwarded to the Safety Manager.

5) Monthly Injury Analysis (OSHA 200 Log) is used to report on-the-job injuries by month. This
will be provided by the Safety Manager.

6) Weekly and Monthly Safety Reports complied by the Safety Manager, reviewed by
Construction Manager and submitted to Owner Safety Manager. Construction shall keep a
copy on file.

7) Weekly Area Safety Assessments, Area Discrepancy Logbooks, Safety Audit Reports,
Supervisor Safety Logbook, Safety Action Plans, Safe Work Plans, Permits, Inspection
Reports, Third Party Certification and all other safety procedural forms, shall be
maintained. Construction Management and Safety Management shall review them regularly

to assist in managing safety and improving safety on the project by taking the necessary
corrective/improvement actions.
